When I was a kid,

the disaster we worried about most was a nuclear war.

That's why we had a barrel like this down in our basement,

When the nuclear attack came,

we were supposed to go downstairs, hunker down, and eat out of that barrel.

Today the greatest risk of global catastrophe

doesn't look like this.

Instead, it looks like this.

If anything kills over 10 million people in the next few decades,

it's most likely to be a highly infectious virus

rather than a war.

Not missiles, but microbes.

Now, part of the reason for this is that

we've invested a huge amount in nuclear deterrents.

But we've actually invested very little in a system to stop an epidemic.

We're not ready for the next epidemic.

Let's look at Ebola.

エボラの例を見てみましょう

I'm sure all of you read about it in the newspaper,

皆さんきっとエボラについて 新聞で読まれたでしょう

lots of tough challenges.

I followed it carefully through the case analysis tools

我々がポリオ撲滅の状況を追った時の

we use to track polio eradication.

And as you look at what went on,

観察していて分かったことは

the problem wasn't that there was a system that didn't work well enough,

システムが不十分で状況に 対処し切れていなかった ということではなく

the problem was that we didn't have a system at all.

そもそもシステム自体すら 存在しなかった ということでした

In fact, there's some pretty obvious key missing pieces.

We didn't have a group of epidemiologists ready to go, who would have gone,

急な召集にも応じ出動できる―そして出動するべきだった 疫学者達の一団はいませんでした

seen what the disease was, seen how far it had spread.

The case reports came in on paper.

It was very delayed before they were put online

and they were extremely inaccurate.

We didn't have a medical team ready to go.

We didn't have a way of preparing people.

Now, Médecins Sans Frontières did a great job orchestrating volunteers.

国境なき医師団は大変うまく ボランティア達を指揮しましたが

But even so, we were far slower than we should have been

それでも私たちは これらの国々へ何千人もの

getting the thousands of workers into these countries.

支援者を送り込むのに 無駄な時間を要したのです

And a large epidemic would require us to have hundreds of thousands of workers.

規模の大きな伝染病が起こると 何十万という支援スタッフが必要となります

There was no one there to look at treatment approaches.

現地では 治療が適切に行われているかを確認する人員すら一人もおらず

No one to look at the diagnostics.

診断方法を見る人も

No one to figure out what tools should be used.

どんなツールが使われるべきか 判断する人もいませんでした

As an example, we could have taken the blood of survivors,

スタッフがいれば 例えば生存者の血液を採取し

processed it, and put that plasma back in people to protect them.

それから作った血清を人々に注射し 免疫を作ってみたりできたでしょう

But that was never tried.

しかしそれは実現されませんでした

So there was a lot that was missing.

つまり 見落とされた事が 数多くあったのです

And these things are really a global failure.

これらは 実に グローバルな規模の失敗です

The WHO is funded to monitor epidemics, but not to do these things I talked about.

WHOには疫病をモニタリングする為の研究費はありますが こうしたことを行うための資金はありません

Now, in the movies it's quite different.

映画は現実とはかなり異なります

There's a group of handsome epidemiologists ready to go,

この美男美女の疫学者達は 常に出動準備万端で

they move in, they save the day, but that's just pure Hollywood.

現場に駆けつけ 救援に成功します でもそれはハリウッドの空想世界です

The failure to prepare could allow the next epidemic

予め対策が準備できていないだけで

to be dramatically more devastating than Ebola

次の疫病がエボラよりも 劇的に危機をもたらすかも知れません

Let's look at the progression of Ebola over this year.

今年エボラが蔓延した段階を 見てみましょう

About 10,000 people died,

およそ１万人が亡くなり

and nearly all were in the three West African countries.

ほぼ全員が西アフリカの ３カ国の住民でした

There's three reasons why it didn't spread more.

これ以上に 感染拡大しなかった理由が３つあります

The first is that there was a lot of heroic work by the health workers.

一つ目は ヘルスワーカー達による 英雄的な努力です

They found the people and they prevented more infections.

エボラ患者を発見し 感染が広がるのを防ぎました

The second is the nature of the virus.

２つめはこのウイルスの性質です

Ebola does not spread through the air.

エボラは空気感染をしません

And by the time you're contagious,

患者が感染源となるまで悪化した頃には

most people are so sick that they're bedridden.

通常症状が重過ぎてベッドから動けません

Third, it didn't get into many urban areas.

３つ目に 感染は都市部に あまり行き着かなかったことです

And that was just luck.

If it had gotten into a lot more urban areas,

もし エボラが都市部に感染拡大していたら

the case numbers would have been much larger.

So next time, we might not be so lucky.

次はそう運に恵まれないかもしれません

You can have a virus where people feel well enough while they're infectious

ウイルスの中には 感染していても症状が無く

that they get on a plane or they go to a market.

そのまま飛行機に乗ったり 市場に行ったりするケースもあります

The source of the virus could be a natural epidemic like Ebola,

ウイルスの感染源はエボラのような 自然由来の疫病だったり

or it could be bioterrorism.

バイオテロでもあり得るのです

So there are things that would literally make things a thousand times worse.

In fact, let's look at a model of a virus spread through the air,

like the Spanish Flu back in 1918.

So here's what would happen:

It would spread throughout the world very, very quickly.

猛烈な勢いで 世界中に瞬く間に蔓延し

And you can see over 30 million people died from that epidemic.

So this is a serious problem.

We should be concerned.

But in fact, we can build a really good response system.

We have the benefits of all the science and technology that we talk about here.

We've got cell phones

to get information from the public and get information out to them.

We have satellite maps where we can see where people are and where they're moving.

We have advances in biology

that should dramatically change the turnaround time to look at a pathogen

and be able to make drugs and vaccines that fit for that pathogen.

So we can have tools,

but those tools need to be put into an overall global health system.

And we need preparedness.

The best lessons, I think, on how to get prepared

are again, what we do for war.

For soldiers, we have full-time, waiting to go.

We have reserves that can scale us up to large numbers.

対策部隊の規模をスケールアップする為の 人員や資源は潤沢にあります

NATO has a mobile unit that can deploy very rapidly.

NATOには非常に迅速に 現地へ赴ける機動部隊がいます

NATO does a lot of war games to check, are people well trained?

NATOは頻繁にウォーゲームを行って確認します 人員が十分に訓練されているか？

Do they understand about fuel and logistics

燃料や物流の状況が理解出来ているか？

and the same radio frequencies?

So they are absolutely ready to go.

そうして人員は常に完璧に 出動可能な状態なのです

So those are the kinds of things we need to deal with an epidemic.

こうしたことが 疫病への対策にも必要なのです

What are the key pieces?

では主なキー・ピースは何でしょう？

First, we need strong health systems in poor countries.

第一に 貧しい国々にしっかりとした 病院・健康保証システムがある事

That's where mothers can give birth safely,

母親が安全に出産できて 子どもたちが必要なワクチンを

kids can get all their vaccines.

全て受けられるような仕組みです

But, also where we'll see the outbreak very early on.

アウトブレイクの初期の兆候も そこで把握出来ます

We need a medical reserve corps:

次に 医療従事者達の 待機部隊が必要です

lots of people who've got the training and background

医療に携わるよう訓練され 経験のある

who are ready to go, with the expertise.

いつでも招集に応じられる 専門知識のある人々です

And then we need to pair those medical people with the military.

そして 彼らを軍隊と連携させます

taking advantage of the military's ability to move fast, do logistics

軍隊の機動力を利用し 物流を確保し 感染地の隔離等を

and secure areas.

素早く行えるようにです

We need to do simulations,

戦争ではなく 細菌の

germ games, not war games, so that we see where the holes are.

拡大シミュレーションをして 対策の為に欠けている要素をあぶり出すのです

The last time a germ game was done in the United States

前回アメリカで細菌シミュレーションが 行われた際は

was back in 2001, and it didn't go so well.

2001年でしたが 良い結果ではありませんでした

So far the score is germs: 1, people: 0.

細菌との戦いは 今のところ人類の負けです

Finally, we need lots of advanced R&D in areas of vaccines and diagnostics.

最後に ワクチンや診断の分野での 研究開発がもっと必要です

There are some big breakthroughs, like the Adeno-associated virus,

例えばアデノウイルス系ワクチンのように 非常に速く効く

that could work very, very quickly.

目覚ましいブレークスルーもあります

Now I don't have an exact budget for what this would cost,

これに必要な 正確な予算はわかりませんが

but I'm quite sure it's very modest compared to the potential harm.

The World Bank estimates that if we have a worldwide flu epidemic,

世界銀行は 世界規模での インフルエンザ大流行が起これば

global wealth will go down by over three trillion dollars

世界の総資産が360兆円以上の 打撃を受けると推定しています

and we'd have millions and millions of deaths.

加えて 何百万人という死者が見込まれます

These investments offer significant benefits

こうした投資は 単に疫病への準備が出来る

beyond just being ready for the epidemic.

という事以上の大きな利益を生みます

The primary healthcare, the R&D,

プライマリケア 研究開発

those things would reduce global health equity

これらは世界的に 健康における公平性を向上させ

and make the world more just as well as more safe.

世界をより 公正で 安全な場所にすることでしょう

So I think this should absolutely be a priority.

ですから 私はこれが 喫緊の課題だと考えます

There's no need to panic.

パニックに陥る必要はありません

We don't have to hoard cans of spaghetti or go down into the basement.

スパゲッティの缶詰を買いだめしたり 地下室へ逃げ込む必要はありません

But we need to get going, because time is not on our side.

でも 時間は味方をしてはくれません 今すぐに 取り組み始めなければ

In fact, if there's one positive thing that can come out of the Ebola epidemic,

実際 エボラの蔓延から得る 何か一つ良い教訓があるとすれば

it's that it can serve as an early warning, a wake-up call, to get ready.

私たちが準備を始める為の 警鐘となったということでしょう

If we start now, we can be ready for the next epidemic.

今始めれば 次の疫病への対策は間に合います
