Some of us will see it coming.

Some of us won't.

There is a little-known documented medical term

突然おとずれる悲運を意味する

called impending doom.

インペンディング・ドゥームという言葉があります

It's almost a symptom.

これはもう症状のようなものです

As a medical provider, I'm trained to respond

私は医療従事者として 様々な症状に

to this symptom like any other,

対応できるよう普段から訓練されていますから

so when a patient having a heart attack looks at me

心臓発作の患者が僕の目を見て

and says, "I'm going to die today,"

「私は今日死ぬんですよね」と言ったとしても

we are trained to reevaluate the patient's condition.

患者の状態を再評価できるよう 訓練されているのです

Throughout my career, I have responded

クリティカルケアの仕事を通して―

to a number of incidents where the patient

この患者はあと数分しかもたないという場面に

had minutes left to live

居合わせたことが何度もあります

and there was nothing I could do for them.

そういうとき 本当に何も手の施しようがないんです

With this, I was faced with a dilemma:

そんなとき向き合わなければならないのは

Do I tell the dying that they are about to face death,

本人に自分が死ぬんだということを告げるべきか

or do I lie to them to comfort them?

それとも嘘をついて慰めを言ってあげるべきか というジレンマです

Early in my career, I faced this dilemma

この仕事をやり始めて間もない頃

by simply lying.

私は単純に嘘をつくことを選んでいました

I was afraid.

それは怖かったからです

I was afraid if I told them the truth,

本当のことを伝えたら

that they would die in terror, in fear,

その人は恐怖のなかで死ぬことになるだろうと

just grasping for those last moments of life.

怖くて仕方がない状態で 人生の終わりを迎えることになると

That all changed with one incident.

ところがある出来事を通して すべてが変わりました

Five years ago, I responded to a motorcycle accident.

５年前 バイク事故の対応をした時のことです

The rider had suffered critical, critical injuries.

バイクに乗っていた人は致命傷を負っていました

As I assessed him, I realized that there was nothing

その患者を診たとき 何をどうしたとしても

that could be done for him,

助けることはできないことは明白でした

and like so many other cases, he looked me in the eye

それまでの患者と同じように その人も私の目を見て

and asked that question: "Am I going to die?"

「僕は死ぬんでしょうか?」と訊きました

これまでとは違う選択をしよう

In that moment, I decided to do something different.

その瞬間 真実を伝えようと思いました

I decided to tell him the truth.

I decided to tell him that he was going to die

彼がこれから死ぬんだということを

and that there was nothing I could do for him.

そして私にはどうすることもできないことを 伝えることにしたんです

His reaction shocked me to this day.

彼の反応に私は大変ショックを受けました

He simply laid back and had a look

そのことは今でも忘れられません

of acceptance on his face.

彼は安らかに 自分の死を受け入れたという顔つきでした

He was not met with that terror or fear

私が想像していたような恐怖や不安は

that I thought he would be.

彼の表情には存在しなかったのです

He simply laid there, and as I looked into his eyes,

ただ静かに横たわる彼の目を見たとき

I saw inner peace and acceptance.

心の平安と運命を受け入れているのだ ということが理解できました

From that moment forward, I decided

その瞬間私は決心しました

it was not my place to comfort the dying with my lies.

嘘をついて慰めようとするのは 私がすべきことではないと

Having responded to many cases since then

それからも 患者の命の最期の瞬間に

where patients were in their last moments

何度も立ち会いましたが―

and there was nothing I could do for them,

ほとんどの場合先ほどと同様に

in almost every case,

施す手はありませんでしたが

they have all had the same reaction to the truth,

どの人も真実を伝えると同じように反応しました

of inner peace and acceptance.

そこにあったのは 心の平安と運命の受容でした

In fact, there are three patterns

これまで数多くこういう場面を経験してきて

I have observed in all these cases.

パターンが三つあることに気付きました

The first pattern always kind of shocked me.

いつ遭遇してもショックを受けてしまう 一つ目のパターンが

Regardless of religious belief or cultural background,

宗教とか文化的な背景に関係なく

there's a need for forgiveness.

許しを乞う というパターン

Whether they call it sin

人によって罪と認識していたり

or they simply say they have a regret,

後悔だったりしますが

their guilt is universal.

罪悪感というのは全世界共通のものなんです

I had once cared for an elderly gentleman

かつて年配の男性をケアしました

who was having a massive heart attack.

大変深刻な心臓発作でした

As I prepared myself and my equipment

今にも心停止するかという状況で

for his imminent cardiac arrest,

私も覚悟して機器の準備を進めました

I began to tell the patient of his imminent demise.

そして その瞬間が すぐそこまで来ていることを告げました

He already knew by my tone of voice and body language.

すると彼は私の声のトーンや 身振りからすでに察していたのです

As I placed the defibrillator pads on his chest,

胸に除細動器のパッドを当てて

prepping for what was going to happen,

迫りくる瞬間に備えていたとき

he looked me in the eye and said,

彼は私の目を見てこう言いました

"I wish I had spent more time with my children and grandchildren

「もっと子供たちや孫たちと過ごせばよかった―

instead of being selfish with my time."

自分のために時間を使いすぎた」 と

Faced with imminent death,

死の瞬間を目前にして

all he wanted was forgiveness.

彼が求めたものは 許し でした

The second pattern I observe

次に 二つ目のパターンですが

is the need for remembrance.

二つ目は 忘れないでほしい という願いです

Whether it was to be remembered in my thoughts

私の思いの中や

or their loved ones', they needed to feel

愛する人々の思いの中で

that they would be living on.

人は決して滅びることはないということを

There's a need for immortality

愛する人々の心の中や思いの中に

within the hearts and thoughts of their loved ones,

それこそ 私やスタッフ 周りにいる人なら誰でもいいから

myself, my crew, or anyone around.

自分はこれからもずっと誰かの心の中に 生き続けることを確信したいのです

Countless times, I have had a patient look me in the eyes

数え切れないくらいの多くの患者から 僕のじっと目を見て

and say, "Will you remember me?"

「私のことを忘れないでほしい」と言われました

The final pattern I observe

最後に三つ目のパターンです

always touched me the deepest, to the soul.

毎回 私の心にずっしりと重くのしかかる 魂に深く響くパターンなのですが

The dying need to know that their life had meaning.

死に際に彼らがどうしても知りたいこととは

They need to know that they did not waste their life

それは彼らの人生には意味があり けっして無駄に人生を過ごしたのではない

on meaningless tasks.

決して意味のないことに 一生を費やしたのではないということなのです

This came to me very, very early in my career.

この仕事を始めたばかりの早い時期に

I had responded to a call.

電話で駆けつけた ある案件がありました

There was a female in her late 50s

交通事故で車が高速で激突したため

severely pinned within a vehicle.

She had been t-boned at a high rate of speed,

体が車の中で がんじがらめ になっていました

critical, critical condition.

絶体絶命の状況でした

As the fire department worked to remove her from the car,

消防隊が駆けつけて 体を車から出す作業を始めてから

I climbed in to begin to render care.

私はなんとか処置をしようと 車によじ登りました

As we talked, she had said to me,

その時車の中の彼女が言いました

"There was so much more I wanted to do with my life."

「もっともっと生きているうちに やりたいことが一杯あったのに」

She had felt she had not left her mark on this Earth.

自分が生きていたという証を残せなかったと 彼女は感じたのです

As we talked further, it would turn out

話を続けるうちに彼女には

that she was a mother of two adopted children

二人 養子の子どもがいて 二人とも

who were both on their way to medical school.

もうすぐ医大に進学する予定である ということがわかりました

Because of her, two children

この二人の子どもは 彼女がいなかったら決して―

had a chance they never would have had otherwise

手にすることができなかったチャンスを 彼女のおかげで手に入れることができた

and would go on to save lives in the medical field

そして医者になって人の命を助けるために

as medical doctors.

医学の道へ進むことにしたのです

It would end up taking 45 minutes

彼女を車から助け出そうと力を尽くしましたが

to free her from the vehicle.

However, she perished prior to freeing her.

それがかなわぬうちに 彼女は息絶えました

I believed what you saw in the movies:

いろいろな映画で目にする人の最期は

when you're in those last moments

恐怖や不安ばかりなので

that it's strictly terror, fear.

死ぬ瞬間とはそういうものなのだと思っていました

I have come to realize, regardless of the circumstance,

しかし 状況がどうであれ

it's generally met with peace and acceptance,

その人の最期という

that it's the littlest things, the littlest moments,

ほんの短い時間の中で やってあげられる本当に小さなこと

the littlest things you brought into the world

その人を安心させてあげるという 取るに足らないような小さなことこそ

that give you peace in those final moments.

人の最期に平安と運命の受容を もたらすのだ と思い知ったのです
