We are here today

have defined goals

قد حددت اهدافاً

for the progress of countries.

لتقدم الدول

They're called Millennium Development Goals.

تدعى الاهداف الانمائية للألفية

And the reason I really like these goals

وسبب انني احب هذه الاهداف

is that there are eight of them.

هو انها ثمانية اهداف

And by specifying eight different goals,

وكل واحدة منها يحدد مجالاً معيناً

the United Nations has said

وتقول الامم المتحدة

that there are so many things needed

ان هنالك الكثير من الاشياء التي يجب القيام بها

to change in a country

لتغير دولة ما

in order to get the good life for people.

لكي يتحسن وضع معيشة قاطنيها

Look here -- you have to end poverty,

فعليها .. ان تنهي الفقر

education, gender,

وان تنشر التعليم .. وتلغي التميز ضد النساء

child and maternal health,

وان ترفع الصحة .. وان تخفض معدل وفيات الاطفال

control infections, protect the environment

وان تتحكم بانتشار الامراض المعدية .. وان تحمي البيئة

and get the good global links between nations

وان تحرص على التواصل الملائم بين الدول المحيطة بها

in every aspect

في كل المفاهيم

from aid to trade.

سواء كان التواصل ذلك بالمساعدات .. او عن طريق تبادل البضائع

There's a second reason I like these development goals,

وهنالك سبب آخر يجعلني احب هذه الاهداف الانمائية

and that is because each and every one is measured.

وهو ان جميعها يتم قياسه

Take child mortality;

لنأخذ معدل وفيات الاطفال على سبيل المثال

the aim here is to reduce child mortality

ان الهدف هو خفض معدل وفيات الاطفال

by two-thirds,

الى الثلثين

from 1990 to 2015.

من عام 1990 الى عام 2015

That's a four percent reduction per year --

اي خفض 4% كل عام

and this, with measuring.

وهذا وبالقياس

That's what makes the difference

هو ما يفرق

between political talking like this

الكلام السياسي هكذا

and really going for the important thing,

والقيام بالامر الصحيح

a better life for people.

لتحسين حياة الافراد

And what I'm so happy about with this

وانا سعيد جداً حيال هذا الامر

is that we have already documented

اقصد اننا نقوم بتوثيق كل شيء

that there are many countries

وهنالك عدة دول

in Asia, in the Middle East,

في اسيا وفي الشرق الاوسط

in Latin America and East Europe

وفي امريكا اللاتينية وفي اوروبا الشرقية

that [are] reducing with this rate.

قد خفضت معدل وفيات الاطفال

And even mighty Brazil is going down with five percent per year,

فالبرازيل خفضت نسبة وفيات الاطفال بمعدل 5% كل عام

and Turkey with seven percent per year.

وتركيا 7% كل عام

So there's good news.

وهذه اخبار رائعة جداً

But then I hear people saying, "There is no progress in Africa.

ولكن هنالك من يقول .. ولكن لا يوجد اي تقدم في افريقيا

And there's not even statistics on Africa

ولا يوجد حتى احصائيات في افريقا

to know what is happening."

لكي نعلم مالذي يجري هناك

I'll prove them wrong on both points.

And I will take Kenya as an example.

Here you see the data.

Don't panic -- don't panic now, I'll help you through this.

It looks nasty, like in college

when you didn't like statistics.

But first thing, when you see dots like this,

you have to ask yourself:

from where do the data come?

What is the origin of the data?

اين هو مصدرها .. وما هو اساسها

Is it so that in Kenya,

هل في كينيا

there are doctors and other specialists

يقوم الاطباء والمختصون

who write the death certificate at the death of the child

الذين يوقعون شهادات وفاة الاطفال

and it's sent to the statistical office?

برفع تقاريرهم الى مراكز الاحصاء

No -- low-income countries like Kenya

لا .. ففي الدول ذات الدخل المنخفض مثل كينيا

still don't have that level of organization.

لا تملك نظم مثل تلك ..

It exists, but it's not complete

لديها هذا النظام .. ولكنه ليس مكتمل بعد

because so many deaths occur in the home

لان معظم الوفيات تحدث في المنازل

with the family,

ومعظم العائلات

and it's not registered.

لا تكون في الاساس مسجلة في السجلات المدنية

What we rely on is not an incomplete system.

ونحن لا نعتمد عادة على نُظم غير مكتملة

We have interviews, we have surveys.

And this is highly professional

female interviewers

حيث تقوم نسوة بعمل مقابلات

who sit down for one hour with a woman

وتجلس مع النسوة الاخرى لمدة ساعة كاملة

and ask her about [her] birth history.

تسئلها عن تاريخ ولاداتها

How many children did you have?

وعن عدد الاطفال الذين انجبت

Are they alive?

وعن كونهم على قيد الحياة او لا

If they died, at what age and what year?

وان كان منهم من لاقى حتفه .. فمتى تم ذلك ؟

And then this is done in a representative sample

ويتم القيام بتلك المقابلات على عينات تمثل شريحة المجتمع

of thousands of women in the country

من عدة الالاف من النسوة في كينيا

and put together in what used to be called

ومن ثم يتم تجميع هذه المعلومات في ما يسمى

a demographic health survey report.

تقرير استقصاء ديموغرافي صحي

But these surveys are costly,

وهذه الاستقصاءات مكلفة جداً

so they can only be done [in] three- to five-year intervals.

وتحتاج من 3-5 سنوات لاتمامها

But they have good quality.

ولكنها تنتج معلومات عالية الجودة

So this is a limitation.

ولكنها محدودة

And all these colored lines here are results;

each color is one survey.

But that's too complicated for today, so I'll simplify it for you,

and I give you one average point for each survey.

This was 1977, 1988,

And when the experts in the U.N.

have got these surveys in place in their database,

then they use advanced mathematical formulas

to produce a trend line, and the trend line looks like this.

But watch out --

they continue the line

beyond the last point

out into nothing.

And they estimated that in 2008,

ويتوقع القيم في عام 2008

Kenya had per child mortality of 128.

ان يكون معدل وفيات الاطفال 128

And I was sad,

وهذا أمرٌ محزن لي

because we could see

لاننا نرى

this reversal in Kenya

انه في التسعينات انعكست الآية

with an increased child mortality in the 90s.

واصبح معدل الوفيات يرتفع بدلاً من الانخفاض

It was so tragic.

لقد كان امراً مفزعاً

But in June, I got a mail in my inbox

ولكن في شهر يونيو .. وصلني في بريدي

from Demographic Health Surveys,

من الاستقصاء الديموغرافي الصحي

and it showed good news from Kenya.

انباء جيدة من كينيا

I was so happy.

لقد كنت سعيداً جداً

This was the estimate of the new survey.

لقد كان هنالك تقدير جديد تبعاً للاستقصاء

Then it just took another three months

واستغرق الامر ثلاثة اشهر لادخال معلوماته في بيانات

for [the] U.N. to get it into their server,

الامم المتحدة

and on Friday we got the new trend line --

وفي يوم الجمعة الماضي حصلنا على منحى جديد

it was down here.

اصبح يهبط هكذا ..

Isn't it nice -- isn't it nice, yeah?

اليس هذا رائعاً .. رائعٌ جداً اليس كذلك

I was actually, on Friday, sitting in front of my computer,

لقد كنت يوم الجمعة اجلس امام جهاز الكمبيوتر خاصي

and I saw the death rate fall

ورأيت كيف انخفض معدل وفيات الاطفال

from 128 to 84 just that morning.

من 128 الى 84 خلال ذلك الصباح

So we celebrated.

لقد احتفلنا حينها

But now, when you have this trend line,

حسناً عندما نملك هذا المنحى

how do we measure progress?

كيف يمكننا قياس التقدم ؟

I'm going into some details here,

because [the] U.N. do it like this.

لان الامم المتحدة تقوم بهذا الشيء

They start [in] 1990 -- they measure to 2009.

انها تبدأ من عام 1990 .. وتقيس التقدم الى عام 2009

They say, "0.9 percent, no progress."

وتقول انه 0.9 .. ليس تقدم جيد

That's unfair.

ولكن هذا غير عادل

As a professor, I think I have the right to propose something differently.

كبروفسور .. اعتقد ان لدي الحق بان اعترض على هذا

I would say, at least do this --

اريد ان اقول .. على الاقل قوموا بالامر على هذا المنحى

10 years is enough to follow the trend.

It's two surveys, and you can see what's happening now.

انه استقصاءان .. يمكننا ان نرى ماذا يحدث هنا

They have 2.4 percent.

Had I been in the Ministry of Health in Kenya,

هل كنت اعمل مع وزارة الصحة في كينيا !

I may have joined these two points.

ربما ضممت هاتين النقطتين

So what I'm telling you

ما اخبركم اياه

is that we know the child mortality.

اننا نعي انه بالنسبة لمعدل وفيات الاطفال

We have a decent trend.

فانه لدينا منحى جيد

It's coming into some tricky things then

لديه العديد من الخفايا

when we are measuring MDGs.

ولكن عندما نقيس تحقيق الاهداف الانمائية للالفية

And the reason here for Africa is especially important,

علينا أن ننظر الى افريقيا بعين مختلفة

because '90s was a bad decade,

لان التسعينات كانت عقداً عصيباً في افريقيا

not only in Kenya, but across Africa.

ليس على كينيا فحسب .. بل على كل افريقيا

The HIV epidemic peaked.

حيث انتشر وباء نقص المناعة المكتسبة

There was resistance for the old malaria drugs, until we got the new drugs.

وكان هنالك ممانعة ضد لقاحات الملاريا .. وانتظرنا قدوم اللقاحات الجديدة

We got, later, the mosquito netting.

ومن ثم وصول شبكات البعوض

And there was socio-economic problems,

وكان هنالك مشاكل اجتماعية اقتصادية كبيرة

which are now being solved at a much better scale.

وقد تم حل معظمها اليوم على نطاق واسع

So look at the average here --

لذا علينا ان ننظر الى المعدل هنا

this is the average for all of sub-Saharan Africa.

هذا هو معدل أفريقيا جنوب الصحراء

And [the] U.N. says

والامم المتحدة تقول

it's a reduction with 1.8 percent.

ان نسبة خفض معدل وفيات الاطفال كانت 1.8

Now this sounds a little theoretical,

ولكن هذا نظريٌ بعض الشيء ..

but it's not so theoretical.

انه ليس نظريٌ جداً

You know, these economists,

ولكن علينا ان نعي .. ان الاقتصادين

they love money, they want more and more of it, they want it to grow.

يحبون الاموال .. ويحبون الكثير منها .. ويريدون النمو دوما

So they calculate the percent annual growth rate of [the] economy.

لذا هم يحسبون معدل النمو السنوي للاقتصاد

We in public health, we hate child death,

ونحن في الصحة العامة .. نكره وفيات الاطفال

so we want less and less and less of child deaths.

فنريد نسب الخفض .. والانخفاض لذلك المعدل

So we calculate the percent reduction per year,

لذا قمنا بحساب نسبة الانخفاض السنوية

but it's sort of the same percentage.

وهي نوعا ما نفس النسبة السابقة

If your economy grows with four percent,

وقلنا اذ نمى الاقتصاد بنسبة 4%

you ought to reduce child mortality four percent;

يجب ان ينخفض معدل وفيات الاطفال بنسبة 4%

if it's used well and people are really involved

ان قام كل شخص بما يتوجب عليه من عمل

and can get the use of the resources in the way they want it.

واذا تم استغلال الموارد بالصورة الامثل

So is this fair now to measure this over 19 years?

لذا هل من المنطقي ان قوم بقياس ذلك المعدل على 19 عاماً ؟

An economist would never do that.

ان الاقتصادين لن يقوموا بذلك على الاطلاق

I have just divided it into two periods.

بل سوف يقومون بتقسيمه الى نصفين كما سأفعل

In the 90s, only 1.2 percent,

في التسعينات كان معدل الانخفاض 1.2

only 1.2 percent.

فقط 1.2

Whereas now, second gear --

ولكن اليوم .. وعلى السرعة الثانية

it's like Africa had first gear,

كانت افريقيا في التسعينات تسير على السرعة الاولى

now they go into second gear.

واليوم هم على السرعة الثانية

But even this

وحتى هذه

is not a fair representation of Africa,

النسبة لا تعبر عن افريقيا

because it's an average,

لانها نسبة متوسطة

it's an average speed of reduction in Africa.

And look here when I take you into my bubble graphs.

Still here,

Here we have [the] year.

And I'm now giving you a wider picture than the MDG.

I start 50 years ago

when Africa celebrated independence in most countries.

عندما احتفلت معظم دول افريقيا باستقلالها

I give you Congo, which was high,

سوف اعرض الكونغو في الاعلى

Ghana -- lower. And Kenya -- even lower.

و غانا .. وكينيا في الاسفل

And what has happened over the years since then? Here we go.

لننظر مالذي حدث خلال تلك الفترة الزمنية .. لننطلق

You can see, with independence, literacy improved

كما ترون مع الاستقلال .. تحسن التعليم

and vaccinations started, smallpox was eradicated,

وتم انتشار اللقاحات .. وتم القضاء على الجدري

hygiene was improved, and things got better.

وتحسنت الصحة العامة .. واخذت الامور تأخذ مجرى جيداً

But then, in the '80s, watch out here.

ولكن في الثمانينات .. انظروا هنا

Congo got into civil war,

دخلت الكونغو في حرب اهلية

and they leveled off here.

فتوقفت هنا

Ghana got very ahead, fast.

وهنا اصبحت غانا تسرع الخُطى

This was the backlash in Kenya, and Ghana bypassed,

وكينيا قامت بحركة عكسية .. فتقدمت غانا

but then Kenya and Ghana go down together --

ومن ثم هبط معدل وفيات الاطفال في غانا وكينيا معاً

still a standstill in Congo.

ولكنه بقي متوقفاً في الكونغو

That's where we are today.

وهي ارقام اليوم

You can see it doesn't make sense

كما ترون من غير المنطقي

to make an average of this zero improvement

ان نأخذ معدل التطور المعدوم هنا

and this very fast improvement.

مع هذا التطور الكبير

Time has come

لقد جاء الوقت

to stop thinking about sub-Saharan Africa as one place.

الذي يجب فيه التوقف عن التفكير بأفريقيا جنوب الصحراء ككتلة واحدة

Their countries are so different,

ان الدول مختلفة كثيراً عن بعضها البعض

and they merit to be recognized in the same way,

ومن المجحف ان يتم النظر اليها بمنظار وحيد

as we don't talk about Europe as one place.

فنحن لا نتحدث عن اوروبا وكأنها مكان واحد

I can tell you that the economy in Greece and Sweden are very different --

استطيع جازماً ان اخبركم ان هنالك فروقٌ جمة بين اقتصاد اليونان والسويد

everyone knows that.

الجميع يعلم هذا

And they are judged, each country, on how they are doing.

وكل دولة منها يتم قياس ادائها على حدى

So let me show the wider picture.

دعوني اريكم صورة أوسع

My country, Sweden:

هذه هي موطني ، دولة السويد

1800, we were up there.

في عام 1800 .. كنا هنا فيما يتعلق في معدل وفيات الاطفال

What a strange personality disorder we must have,

يالها من مفارقة كبيرة كنا نملكها

counting the children so meticulously in spite of a high child death rate.

ان نحسب كل وفيات الاطفال التي تحدث بالرغم من رقمها المرتفع جداً

It's very strange. It's sort of embarrassing.

انه امرٌ غريب جداً .. ومحرج ايضاً

But we had that habit in Sweden, you know,

ولكن في السويد هذه هي عادتنا ..

that we counted all the child deaths,

نحن نحسب معدل وفيات الاطفال

even if we didn't do anything about it.

حتى لو كنا لن نقوم بشيء حيالها

And then, you see, these were famine years.

هنا كما ترون كان سنوات مجاعة

These were bad years, and people got fed up with Sweden.

وكانت سنوات عصيبة .. وكان الناس يتململون للحصول على الحرية

My ancestors moved to the United States.

وانتقل بعض اجدادنا الى الولايات المتحدة الامريكية

And eventually, soon they started to get better and better here.

وفي النهاية .. بدأت الامور تتحسن اكثر فاكثر

And here we got better education, and we got health service,

بدأنا نحصل على تعليم افضل .. وخدمات صحية اكثر

and child mortality came down.

واخذ معدل وفيات الاطفال ينخفض

We never had a war; Sweden was in peace all this time.

ولم نخض اي حروب .. كان السويد دولة مسالمة طيلة ذلك الوقت

But look, the rate of lowering

انظروا الى نسبة الانخفاض

in Sweden

في السويد

was not fast.

لم تكن سريعة جداً

Sweden achieved a low child mortality

لقد حققت السويد نسبة منخفضة لمعدل وفيات الاطفال

because we started early.

لاننا بدأنا مبكراً

We had primary school actually

فقد حصلنا على التعليم الابتدائي

started in 1842.

في عام 1842

And then you get that wonderful effect

وكان لهذا وقعٌ كبير على ذلك المعدل

when we got female literacy

وخاصة عندما بدأنا تعليم الاناث

one generation later.

وبعد جيل واحد

You have to realize that the investments we do in progress

are long-term investments.

It's not about just five years --

And Sweden never reached [the] Millennium Development Goal rate,

3.1 percent when I calculated.

So we are off track -- that's what Sweden is.

But you don't talk about it so much.

We want others to be better than we were, and indeed, others have been better.

Let me show you Thailand,

see what a success story, Thailand from the 1960s --

انظروا الى قصة النجاح هذه .. انها تايلند منذ عام 1960

how they went down here

انخفض معدل وفيات الاطفال لديها بهذه السرعة

and reached almost the same child mortality levels as Sweden.

ووصلت تقريبا الى نفس معدل السويد

And I'll give you another story -- Egypt,

انظروا الى قصة أخرى .. الى مصر مثلاً

the most hidden, glorious success in public health.

انه الانجاز العظيم المتخفي في الصحة العامة

Egypt was up here in 1960,

لقد كانت هنا مصر في 1960

higher than Congo.

كان معدل الوفيات للاطفال لديها اكبر من الكونغو

The Nile Delta was a misery for children

وكانت دلتا النيل اسوء مكان للاطفال

with diarrheal disease

فقد كانت امراض الاسهال تفتك بهم

and malaria and a lot of problems.

والملاريا تسبب الكثير من المشاكل

And then they got the Aswan Dam. They got electricity in their homes,

ولكن ومع بناء سد اسوان .. ومع دخول الكهرباء الى المنازل

they increased education

ونمو التعليم

and they got primary health care.

والحصول على الخدمات الصحية الاساسية

And down they went, you know.

هبط معدل وفيات الاطفال الى هنا .. كما ترون

And they got safer water, they eradicated malaria.

عندما حصلوا على الماء النقي .. وتم محاربة الملاريا

And isn't it a success story.

Millennium Development Goal rates for child mortality

is fully possible.

And the good thing is

والامر الجيد

that Ghana today is going with the same rate

ان غانا اليوم تسير بنفس السرعة

as Egypt did at its fastest.

التي سارت بها مصر من قبل

Kenya is now speeding up.

وكينا اليوم تسارع الخُطى ايضا

Here we have a problem.

ولكن هنا لدينا مشكلة

We have a severe problem in countries which are at a standstill.

مكشلة كبيرة جداً .. في مثل هذه الدول التي لا تتحسن

Now, let me now bring you to a wider picture,

a wider picture of child mortality.

I'm going to show you the relationship

between child mortality on this axis here --

and here I have the family size.

The relationship between child mortality and family size.

One, two, three, four children per woman:

six, seven, eight children per woman.

This is, once again, 1960 --

50 years ago.

Each bubble is a country --

the color, you can see, a continent.

They had high, or very high, child mortality

And the ones over there,

they were so-called Western countries.

They had low child mortality

and small families.

What has happened?

What I want you [to do] now is to see with your own eyes

the relation between fall in child mortality

and decrease in family size.

It's the richest country in the world with Al Jazeera.

انها اغنى دولة في العالم .. وتملك قناة الجزيرة

How the heck could they be [a] developing country?

كيف بحق الإله تصنف كدولة نامية ؟

يمكننا ان نرى بوضوع كيف ان معدل وفيات الاطفال ينخفض

and decreasing family size,

مع انخفاض عدد افراد الاسرة

even within Africa.

It's very clear that this is what happens.

وهذا جليٌ جداً اليوم

And a very important piece of research came out on Friday

وهنالك بحثٌ جديد ظهر يوم الجمعة

from the Institute of Health Metrics and Evaluation in Seattle

من معهد القياسات والتقيم الصحي في سياتل

showing that almost 50 percent

يقول ان 50% تقريباً

of the fall in child mortality

من انخفاض معدل وفيات الاطفال

can be attributed to female education.

يرجع لتعليم الفتيات

That is, when we get girls in school,

اذا عندما ندخل الاناث الى المدارس

we'll get an impact 15 to 20 years later,

سوف نحصل على النتائج بعد 15-20 عام قادمة

which is a secular trend which is very strong.

وهذا سينتج منحة جيدٌ جداً وقوي باتجاه خفض ذلك المعدل

That's why we must have that long-term perspective,

وهذا هو الذي يفرض علينا العمل بمفاهيم طويلة الامد

but we must measure the impact

ولكن علينا ان نقيس النتائج

over 10-year periods.

كل 10 سنوات

It's fully possible

انه امرٌ ممكن حدوثه

to get child mortality down in all of these countries

ان نخفض معدل وفيات الاطفال

and to get them down in the corner

وان ندفع الدول الى المربع هذا في هذه الزاوية

where we all would like to live together.

حيث نريد ان نعيش جميعاً معاً هنا

And of course, lowering child mortality

وحتماً خفض معدل وفيات الاطفال

is a matter of utmost importance

هو امرٌ مهم جداً

from humanitarian aspects.

من الجانب الانساني..

It's a decent life for children,

انه امرٌ واجب .. ان نعطي للاطفال حياة لائقة

we are talking about.

وهو الذي نتحدث عنه الان

But it is also a strategic investment

ولكن الامر استثمار استراتيجي ايضاً

in the future of all mankind,

لكل البشرية .. ولمستقبلها

because it's about the environment.

لانه متعلق بالبيئة ايضاً

We will not be able to manage the environment

فنحن لن نستطيع ان نتحكم بالمشاكل البيئية

and avoid the terrible climate crisis

وتجنب التغيرات والازمات المناخية

if we don't stabilize the world population.

ان لم نستطيع ان نسيطر على النمو السكاني .. وعدد سكان كوكب الارض

Let's be clear about that.

لنكن واضحين في هذا الخصوص

And the way to do that,

ولكي نقوم بذلك

that is to get child mortality down, get access to family planning

علينا ان نخفض معدل وفيات الاطفال .. وان نعمل على تنظيم النسل وتحديده

and behind that drive female education.

وكل هذا بالتوازي مع نشر تعليم الاناث

And that is fully possible. Let's do it.

وهذا كله أمرٌ ممكن حدوثه .. وعلينا القيام به
