David Sengeh: The sore problem of prosthetic limbs

I was born and raised in Sierra Leone,

ഞാന്‍ ജനിച്ചതും വളര്‍ന്നതും സിയെറ ലിയോണിലാണ്

a small and very beautiful country

വളരെ ചെറുതും മനോഹരവുമായ ഒരു രാജ്യം

in West Africa,

പടിഞ്ഞാറന്‍ ആഫ്രിക്കയിലാണിത്

a country rich both in physical resources

പ്രകൃതിവിഭവങ്ങള്‍ കൊണ്ടും സൃഷ്ടിപരമായ പ്രാഗല്ഭ്യം, കൊണ്ടും

and creative talent.

സമ്പന്നമായ ഒരു രാജ്യം

However, Sierra Leone is infamous

എന്നാല്‍ സിയെറ ലിയോണിനെ കുപ്രസിദ്ധമാക്കിയത്

for a decade-long rebel war in the '90s

തൊണ്ണൂറുകളിലെ വിമത കലാപമാണ്

when entire villages were burnt down.

ഗ്രാമങ്ങള്‍ പലതും തന്നെ കത്തി ചാമ്പലായി.

An estimated 8,000 men, women and children

had their arms and legs amputated during this time.

As my family and I ran for safety

when I was about 12 from one of those attacks,

I resolved that I would do everything I could

to ensure that my own children

would not go through the same experiences we had.

They would, in fact, be part of a Sierra Leone

where war and amputation

were no longer a strategy for gaining power.

As I watched people who I knew, loved ones,

ഇങ്ങനെ എനിക്കറിയാവുന്നവര്‍ എനിക്ക് വേണ്ടപ്പട്ടവര്‍

recover from this devastation,

ഈ കൊടിയനാശത്തില്‍ നിന്നും മുക്തരായവരെയെല്ലാം കാണുമ്പോള്‍

one thing that deeply troubled me

ഒരു കാര്യം എന്നെ ശരിക്കും അസ്വസ്ഥനാക്കിയത്

was that many of the amputees in the country

ഇവരാരും തന്നെ അവരുടെ കൃത്രിമാവയവങ്ങള്‍

would not use their prostheses.

ഉപയോഗിക്കുന്നില്ല എന്നുള്ളതാണ്

The reason, I would come to find out,

ഇതിന്റെ കാരണം, ഞാന്‍ മനസ്സിലാക്കിയിടത്തോളം,

was that their prosthetic sockets

were painful because they did not fit well.

The prosthetic socket is the part

in which the amputee inserts their residual limb,

അംഗഭംഗം വന്നവര്‍ അവശേഷിക്കുന്ന ശരീര ഭാഗം തിരുകിവെയ്കുന്നത്.

and which connects to the prosthetic ankle.

അതാണ് രണ്ട് ഭാഗങ്ങളേയും തമ്മില്‍ ബന്ധിപ്പിക്കുന്നത്

Even in the developed world,

വികസിത രാജ്യങ്ങളില്‍ പോലും

it takes a period of three weeks to often years

ആഴ്ചകളും മാസങ്ങളുമെടുത്താണ് പലപ്പോഴും

for a patient to get a comfortable socket, if ever.

Prosthetists still use conventional processes

like molding and casting

to create single-material prosthetic sockets.

ഒരേ പദാര്‍ത്ഥം കൊണ്ടുള്ള കൃത്രിമാവയവങ്ങള്‍ ഉണ്ടാക്കുന്നത്.

Such sockets often leave intolerable amounts

അങ്ങനെയുള്ള കൃത്രിമാവയവങ്ങളുടെ കുഴികള്‍

of pressure on the limbs of the patient,

സഹിക്കാന്‍ പറ്റാത്ത സമ്മര്‍ദ്ദമാണ് രോഗിയിലുണ്ടാക്കുന്നത്

leaving them with pressure sores and blisters.

അത് അവരുടെ മുറിവ് പഴുക്കുന്നതിന് കാരണമാകുന്നു.

It does not matter

മുറിഞ്ഞ ഭാഗം എത്ര ബലമുള്ളതായാലും കാര്യമില്ല

how powerful your prosthetic ankle is.

If your prosthetic socket is uncomfortable,

you will not use your leg,

and that is just simply unacceptable in our age.

So one day, when I met professor Hugh Herr

about two and a half years ago,

and he asked me if I knew how to solve this problem,

I said, "No, not yet,

but I would love to figure it out."

And so, for my Ph.D. at the MIT Media Lab,

I designed custom prosthetic sockets

quickly and cheaply

I used magnetic resonance imaging

to capture the actual shape of the patient's anatomy,

then use finite element modeling to better predict

the internal stresses and strains

on the normal forces,

and then create a prosthetic socket for manufacture.

We use a 3D printer to create

a multi-material prosthetic socket

which relieves pressure where needed

on the anatomy of the patient.

In short, we're using data

to make novel sockets quickly and cheaply.

In a recent trial we just wrapped up

one of our patients, a U.S. veteran

and worn dozens of legs,

said of one of our printed parts,

"It's so soft, it's like walking on pillows,

and it's effing sexy."

Disability in our age

My hope and desire is that the tools and processes

we develop in our research group

ഞങ്ങള്‍ വികസിപ്പിച്ചെടുത്ത ഉപകരണങ്ങളും പ്രക്രിയകളും കൊണ്ട്

can be used to bring highly functional prostheses

ഏറ്റവും മുന്തിയ കൃത്രിമാവയവങ്ങള്‍

to those who need them.

വൈകല്യമുള്ളവര്‍ക്കായി നിര്‍മ്മിക്കാനാകണമെന്നാണ്

For me, a place to begin healing the souls

of those affected by war and disease

is by creating comfortable and affordable interfaces

for their bodies.

Whether it's in Sierra Leone or in Boston,

I hope this not only restores

അവരുടെ മാനുഷികമായ കഴിവുകളെ

but indeed transforms their sense of human potential.
