The first patient to ever be treated with an antibiotic

was a policeman in Oxford.

On his day off from work,

he was scratched by a rose thorn while working in the garden.

That small scratch became infected.

Over the next few days, his head was swollen

with abscesses,

and in fact his eye was so infected

that they had to take it out,

and by February of 1941,

this poor man was on the verge of dying.

He was at Radcliffe Infirmary in Oxford,

and fortunately for him,

a small team of doctors

led by a Dr. Howard Florey

had managed to synthesize

a drug that had been discovered

but had never actually been used to treat a human,

and indeed no one even knew if the drug would work,

Second day, he was doing much better.

They were starting to run out of penicillin,

across the road to re-synthesize the penicillin from his urine

and give it back to him,

and that worked.

Day four, well on the way to recovery.

나흘째 되던 날에는 완쾌가 멀지 않아 보였습니다. 기적이었죠.

This was a miracle.

Day five, they ran out of penicillin,

하지만 닷새째 페니실린이 바닥났고 안타깝게도 그는 죽고 말았습니다.

and the poor man died.

So that story didn't end that well,

but fortunately for millions of other people,

like this child who was treated again in the early 1940s,

who was again dying of a sepsis,

and within just six days, you can see,

Millions have lived,

and global health has been transformed.

Now, antibiotics have been used

for patients like this,

but they've also been used rather frivolously

in some instances,

for treating someone with just a cold or the flu,

which they might not have responded to an antibiotic,

and they've also been used in large quantities

sub-therapeutically, which means in small concentrations,

to make chicken and hogs grow faster.

Just to save a few pennies on the price of meat,

we've spent a lot of antibiotics on animals,

not for treatment, not for sick animals,

아픈 동물들을 치료하려는 게 아니라, 그저 성장 촉진용으로 말입니다.

but primarily for growth promotion.

Now, what did that lead us to?

자, 그래서 제가 그걸 언급하는 이유는 뭘까요?

Basically, the massive use of antibiotics

간단히 말해, 전 세계적으로 이루어 지는 항생제 오남용은

around the world

has imposed such large selection pressure on bacteria

박테리아에 엄청난 선택압으로 작용해서 이제는 저항력이 문제가 생깁니다.

that resistance is now a problem,

because we've now selected for just

우리는 이제 내성있는 박테리아를 선택했기 때문입니다.

the resistant bacteria.

And I'm sure you've all read about this in the newspapers,

물론 여러분 모두 이런 이야기를 신문에서도 읽었을 것이고,

you've seen this in every magazine

that you come across,

but I really want you to appreciate

the significance of this problem.

This is serious.

The next slide I'm about to show you is of carbapenem resistance in acinetobacter.

Acinetobacter is a nasty hospital bug,

아시네토박터는 아주 지독한 병원균이고, 카르바페넴은 쉽게 말해

and carbapenem is pretty much

the strongest class of antibiotics

that we can throw at this bug.

And you can see in 1999

보시다시피, 1999년의 내성균의 분포입니다.

this is the pattern of resistance,

mostly under about 10 percent across the United States.

미국의 전지역에서 10%도 안됩니다.

Now watch what happens when we play the video.

So I don't know where you live,

but wherever it is, it certainly is a lot worse now

than it was in 1999,

and that is the problem of antibiotic resistance.

It's a global issue

affecting both rich and poor countries,

and at the heart of it, you might say, well,

isn't this really just a medical issue?

If we taught doctors how not to use antibiotics as much,

의사들에게 항생제 사용을 줄이라고 하고,

if we taught patients how not to demand antibiotics,

환자들에게 항생제를 요구하지 말라고 하면 쉽게 해결될 문제라고요.

perhaps this really wouldn't be an issue,

and maybe the pharmaceutical companies

또 제약 회사들이 더욱 열심히 노력해서 새로운 항생제를 개발하면 된다고요.

should be working harder to develop

more antibiotics.

Now, it turns out that there's something fundamental about antibiotics

하지만 항생제는 근본적으로 다른 약들과는 다른 점이 있습니다.

which makes it different from other drugs,

which is that if I misuse antibiotics

제가 항생제를 남용하거나, 심지어 사용하기만 해도

or I use antibiotics,

not only am I affected but others are affected as well,

저뿐만이 아니라 모두가 영향을 받는다는 겁니다.

in the same way as if I choose to drive to work

제가 자가용으로 출근하거나 비행기로 여행하게 되면

or take a plane to go somewhere,

that the costs I impose on others

지구 온난화를 통해 모두에게 피해를 주게 되는 것과 마찬가지입니다.

through global climate change go everywhere,

and I don't necessarily take these costs into consideration.

하지만 저는 이런 피해를 진지하게 고려하지 않겠죠.

This is what economists might call a problem of the commons,

경제학자들은 이런 것을 공유지의 비극이라고 부릅니다.

and the problem of the commons is exactly

그리고 지금 항생제 사용에서 나타나는 현상도 공유지의 비극이죠.

what we face in the case of antibiotics as well:

that we don't consider —

개인, 환자, 병원과 전체 의료 체계를 포함한

and we, including individuals, patients,

hospitals, entire health systems —

do not consider the costs that they impose on others

우리 모두가 항생제의 사용으로 인한 피해를 고려하지 않고 있습니다.

by the way antibiotics are actually used.

Now, that's a problem that's similar

이와 비슷한 문제 양상을 보이는 친숙한 다른 분야가 있습니다.

to another area that we all know about,

which is of fuel use and energy,

바로 연료 소비와 에너지입니다.

and of course energy use

물로 에너지의 소비는 에너지를 고갈시키는 동시에

both depletes energy as well as

leads to local pollution and climate change.

일대를 오염시키고 기후 변화를 일으킵니다.

And typically, in the case of energy,

에너지 문제에 있어서는 보통 두 가지 방법으로 문제를 해결합니다.

there are two ways in which you can deal with the problem.

One is, we can make better use of the oil that we have,

하나는 우리가 가지고 있는 석유를 좀 더 현명하게 쓰는 것이고,

and that's analogous to making better use

이건 지금 우리에게 있는 항생제를 더 지혜롭게 쓰는 것과 유사하죠.

of existing antibiotics,

and we can do this in a number of ways

that we'll talk about in a second,

but the other option is the "drill, baby, drill" option,

which in the case of antibiotics is to go find new antibiotics.

Now, these are not separate.

They're related, because if we invest heavily

in new oil wells,

we reduce the incentives for conservation of oil

in the same way that's going to happen for antibiotics.

The reverse is also going to happen, which is that

반대의 경우도 일어날텐데, 우리가 항생제를 좀 더 적절하게 사용한다면

if we use our antibiotics appropriately,

we don't necessarily have to make the investments

in new drug development.

And if you thought that these two were entirely,

이 두 방법이 서로 완벽하게 균형을 이루고 있다고 생각하신다면,

fully balanced between these two options,

you might consider the fact that

이건 우리가 하는 일종의 게임이라는 점을 고려하셔야 합니다.

this is really a game that we're playing.

The game is really one of coevolution,

공진화라는 이름의 게임입니다.

and coevolution is, in this particular picture,

공진화는, 이 사진을 예로 들면, 치타와 가젤 사이에서 나타납니다.

between cheetahs and gazelles.

Cheetahs have evolved to run faster,

치타는 빨리 달릴 수 있게 진화했죠. 빨리 못달리면 끼니를 거를 테니까요.

because if they didn't run faster,

they wouldn't get any lunch.

Gazelles have evolved to run faster because

가젤도 빨리 달릴 수 있게 진화했는데, 못달리면 먹이거리가 되기 때문입니다.

if they don't run faster, they would be lunch.

Now, this is the game we're playing against the bacteria,

이것은 우리가 박테리아에 대항해서 벌이는 게임입니다.

except we're not the cheetahs,

하지만 여기서 우리는 치타가 아니고 가젤이라는 점이 다르죠.

we're the gazelles,

and the bacteria would,

just in the course of this little talk,

would have had kids and grandkids

and figured out how to be resistant

그렇게 자연 선택과 시행착오로 내성을 기르는 거죠.

just by selection and trial and error,

trying it over and over again.

Whereas how do we stay ahead of the bacteria?

반면 우리는 어떻게 박테리아보다 더 진보하는 걸까요?

We have drug discovery processes,

우리는 새로운 약의 발견 과정이 있고, 분자 검사를 하고,

screening molecules,

we have clinical trials,

임상 시험을 거쳐서, 새로운 후보 약을 만들어내면

and then, when we think we have a drug,

then we have the FDA regulatory process.

FDA 규정에 따라 승인과정을 받아야 합니다.

And once we go through all of that,

이 모든 과정을 거쳐야 박테리아로부터 겨우 한발 앞설까 말까 합니다.

then we try to stay one step ahead

of the bacteria.

Now, this is clearly not a game that can be sustained,

이건 절대 지속 가능한 게임이 아닙니다.

or one that we can win

단순히 혁신을 통해 이길 수 있는 것도 아니고요.

by simply innovating to stay ahead.

We've got to slow the pace of coevolution down,

우리는 공진화의 속도를 늦춰야 합니다.

and there are ideas that we can borrow from energy

또한 에너지에서 아이디어를 빌려올 수도 있어요.

that are helpful in thinking about

항상제의 문제에 관해 어떻게 대처할 지를 생각하는 데 유용하도록 말이죠.

how we might want to do this in the case

of antibiotics as well.

Now, if you think about how we deal with

자, 에너지의 가격 결정 정책을 한번 살펴보자면,

energy pricing, for instance,

we consider emissions taxes,

which means we're imposing the costs of pollution

실제로 에너지를 소비하는 사람들에게 공해 발생 비용을 부담시키는 거죠.

on people who actually use that energy.

We might consider doing that for antibiotics as well,

항생제에도 이런 걸 적용하는 것을 생각해 보게 되는데

and perhaps that would make sure that antibiotics

그것은 항생제가 올바른 방향으로 사용될 수 있도록 해 줄 겁니다.

actually get used appropriately.

There are clean energy subsidies,

청정에너지 보조금은 공해를 덜 발생시키거나

which are to switch to fuels which don't pollute as much

or perhaps don't need fossil fuels.

아예 발생시키지 않는 연료를 사용하도록 장려하는 거죠.

Now, the analogy here is, perhaps we need

이와 유사한 방법을 사용하고자 한다면

to move away from using antibiotics,

항생제를 사용하지 않을 방법을 찾아야 할 겁니다.

and if you think about it, what are good substitutes for antibiotics?

그러면 항생제의 대안에는 무엇이 있을까요?

Well, turns out that anything that reduces

항생제의 필요성을 줄이는 것이라면, 어떤 것이든 가능합니다.

the need for the antibiotic would really work,

so that could include improving hospital infection control

병원 감염 관리나 백신 접종을 포함하여 특히 계절성 독감 백신이 가장 큽니다.

or vaccinating people,

particularly against the seasonal influenza.

And the seasonal flu is probably

계절성 독감백신은 국내외를 막론하고 항생제 사용을 부추기는 큰 요인이죠.

the biggest driver of antibiotic use,

both in this country as well as in many other countries,

and that could really help.

그러니 백신을 맞지 않으면 큰 도움이 되겠죠.

A third option might include something like tradeable permits.

세 번째 방법은 허가증 거래와 같은 것을 도입하는 것입니다.

And these seem like faraway scenarios,

아직은 먼 미래의 얘기인 것처럼 여겨지지만,

but if you consider the fact that we might not

감염으로 항생제가 필요한 모두에게 그것을 줄 여유가 안 된다면

have antibiotics for many people who have infections,

we might consider the fact that we might

다른 사람보다 우선 항생제를 지급 받을 사람들을 선택할 기준을 고려 해야겠죠.

want to allocate who actually gets to use

some of these antibiotics over others,

and some of these might have to be on the basis of clinical need,

but also on the basis of pricing.

비용에 따른 필요도 또한 될 수 있겠죠.

And certainly consumer education works.

소비자 교육은 확실히 효과가 있죠.

Very often, people overuse antibiotics

or prescribe too much without necessarily

knowing that they do so,

and feedback mechanisms

have been found to be useful,

both on energy —

When you tell someone that they're using

a lot of energy during peak hour,

they tend to cut back,

and the same sort of example has been performed

even in the case of antibiotics.

A hospital in St. Louis basically would put up

on a chart the names of surgeons

in the ordering of how much antibiotics they'd used

in the previous month,

and this was purely an informational feedback,

there was no shaming,

but essentially that provided some information back

외과의들에게 항생제의 사용 습관을 재고해볼 만한 정보를 제공한 셈입니다.

to surgeons that maybe they could rethink

how they were using antibiotics.

Now, there's a lot that can be done

공급의 측면에서도 변화의 여지는 많습니다.

on the supply side as well.

페니실린의 가격을 보시면 하루치의 값은 약 10센트로 꽤 저렴한 약이죠.

If you look at the price of penicillin,

the cost per day is about 10 cents.

It's a fairly cheap drug.

If you take drugs that have been introduced since then —

하지만 리네졸리드나 뎁토마이신 같은 페니실린 이후로 도입된 약들을 보면

linezolid or daptomycin —

those are significantly more expensive,

가격이 상당히 더 비쌉니다.

so to a world that has been used to paying 10 cents a day for antibiotics,

the idea of paying 180 dollars per day

seems like a lot.

But what is that really telling us?

That price is telling us

that we should no longer

take cheap, effective antibiotics as a given

싸고 효과적인 항생제를 당연시 하면 안 되는 때가 올 것이며,

into the foreseeable future,

and that price is a signal to us

그 가격은 우리가 보존에 더 큰 관심을 기울여야 할것을 알려주는 거죠.

that perhaps we need to be paying

much more attention to conservation.

That price is also a signal

또한 우리가 이제 대안 기술을 찾아 나서야 한다는 신호이기도 합니다.

that maybe we need to start looking at other technologies,

in the same way that gasoline prices are a signal

마치 휘발유 가격이 전기 자동차의 개발을 촉진하는 것처럼 말이죠.

and an impetus, to, say,

the development of electric cars.

Prices are important signals

비록 가격은 우리가 주목해야 할 중요한 신호지만

and we need to pay attention,

but we also need to consider the fact that

항생제 치고는 이례적으로 느껴지는 이 비싼 가격도

although these high prices seem unusual for antibiotics,

they're nothing compared to the price per day

일부 항암제의 하루 치 가격과는 비교할 수 없이 싸다는 겁니다.

of some cancer drugs,

which might save a patient's life only for a few months or perhaps a year,

항암제는 환자의 죽음을 몇 달, 혹은 1년 정도 미룰 뿐이지만

whereas antibiotics would potentially

항생제는 잠재적으로 환자의 생명을 완전히 구할 수 있으니까요.

save a patient's life forever.

So this is going to involve

결국 이건 인식의 전체적 전환이고 무서운 방향으로의 전환이기도 합니다.

a whole new paradigm shift,

and it's also a scary shift because

in many parts of this country,

이 나라의 많은 지역에서도 그렇고, 세계의 여러 지역에서도

in many parts of the world,

the idea of paying 200 dollars

항생제 치료를 위해 하루 200달러를 낸다는 것은 상상조차 할 수 없거든요.

for a day of antibiotic treatment

is simply unimaginable.

So we need to think about that.

우리가 고민해야할 문제이죠.

Now, there are backstop options,

항생제가 무력화될 경우 대안으로 사용할만한 기술들도 연구되고 있는데,

which is other alternative technologies

that people are working on.

It includes bacteriophages, probiotics,

박테리오파지, 생균제,정족수 감지, 신바이오틱 등을 포함합니다.

quorum sensing, synbiotics.

Now, all of these are useful avenues to pursue,

이 모두가 연구해볼 가치가 있는 분야들이고,

and they will become even more lucrative

새로운 항생제들의 가격이 비싸질수록 더욱 시장 이윤 가치는 높아질 것입니다.

when the price of new antibiotics starts going higher,

and we've seen that the market does actually respond,

and the government is now considering

정부는 새로운 항생제 개발에 보조금을 지급하는 방안을 모색하고 있습니다.

ways of subsidizing new antibiotics and development.

But there are challenges here.

하지만 쉽지는 않겠죠. 문제를 그냥 돈으로 해결하려고 해서는 안 됩니다.

We don't want to just throw money at a problem.

What we want to be able to do

대신, 항생제의 올바른 사용 판매가 장려되는 식으로 투자를 진행해야죠.

is invest in new antibiotics

in ways that actually encourage

appropriate use and sales of those antibiotics,

and that really is the challenge here.

여기서는 그것이 난제입니다.

Now, going back to these technologies,

you all remember the line from that famous

dinosaur film, "Nature will find a way."

We really have to remember that, whatever the technology might be,

You might think, well, this is just a problem

but it turns out that we have the exact same

which is a serious problem in India and South Africa.

Thousands of patients are dying because

and in some instances, even those don't work

Viruses are becoming resistant.

Right now, much of the world depends on

one drug, artemisinin drugs,

essentially to treat malaria.

Resistance to artemisinin has already emerged,

and if this were to become widespread,

that puts at risk

the single drug that we have to treat malaria around the world

in a way that's currently safe and efficacious.

Mosquitos develop resistance.

모기도 내성이 생기고 있죠. 아이가 있으시다면 머릿니에 대해 아실 것이고,

If you have kids, you probably know about head lice,

and if you're from New York City,

뉴욕에서 오셨다면 그곳은 지독한 빈대가 유명하다고 알고 있습니다.

I understand that the specialty there is bedbugs.

So those are also resistant.

이것들도 내성이 생기고 있어요.
