This is an inherited disorder.

And in 1992, I participated in a genetic study,

and found to my horror, as you can see from the slide,

that my ascending aorta was not in the normal range,

Everyone in here will be between 3.2-3.6,

and I was already up at 4.4.

And as you can see, my aorta dilated progressively,

and I got closer and closer to the point where surgery was going to be necessary.

수술이 필요한 상황에

도달하게 됐죠.

The surgery on offer was pretty gruesome.

제가 받아야 할 대동맥 수술은 매우 끔찍했죠 --

마취를 하고, 흉부를 개복한 후

Anesthetize you, open your chest,

put you on an artificial heart and lung machine,

인공심폐기에 연결시키고,

drop your body temperature to about 18 centigrade,

체온을 약 18도까지 떨어뜨리고,

stop your heart, cut the aorta out,

심장을 정지시키고, 대동맥을 잘라내고,

replace it with a plastic valve and a plastic aorta.

플라스틱 판막과 플라스틱 대동맥으로 교체하는

것인데 가장 중요한것은

And most importantly, commit you to a lifetime of anticoagulation therapy.

수술후 평생 동안 주로 와파린 같은 약으로

혈액응고 처치법을 받아야 한다는 것이죠.

Normally, warfarin.

그런 수술을 받을 생각은 별로 달갑지 않았죠.

The thought of the surgery was not attractive.

The thought of the warfarin was really quite frightening.

평생동안 워파린을 먹어야 할

생각은 정말로 두려웠어요.

저는 그래서 "나는 연구개발을 하는 엔지니어야

So I said to myself,

"I'm an engineer, I'm in R&D, this is just a plumbing problem."

이건 단순한 배관 문제니까 난 이 문제를

해결할 수 있고 또 해결할꺼야" "라고 생각했죠.

"I can do this, I can change this."

저는 그래서

So I set out to change the entire treatment for aortic dilation.

대동맥 확장증에 대한 치료 방법을

완전히 바꾸기로 마음 먹었죠.

The project aim is really quite simple.

이 프로젝트의 목적은 아주 간단하죠.

The only real problem with the ascending aorta

마르판 증후군이 상승대동맥에

in people with Marfan syndrome

미치는 유일한 큰 문제는 상승대동맥의

is that it lacks some tensile strength.

인장강도가 약간 떨어지게 만든다는 거죠.

그래서 대동맥 바깥을 간단하게 둘러

So, the possibility exists to simply externally wrap the pipe,

감아서 터지지 못하게 하는 치료가 가능할 수 있죠.

and it would remain stable and operate quite happily.

그러면 대동맥이 더 이상 확장하지 않고 잘 작동하겠죠.

고압 호스 파이프나,

If your high-pressure hose pipe or hydraulic line bulges a little,

고수압 라인이 약간 부풀어 올르면,

부푼 부분의 외부를 테이프로 감으면 돼죠.

you just wrap some tape around it, it really is that simple.

In concept, though not in execution.

The great advantage of an external support, for me,

was that I could retain all of my own bits,

So, where do we start?

This is a sagittal slice through me.

In the middle, you can see that little structure squeezing out,

that's the left ventricle,

pushing blood out through the aortic valve.

You can see two of the leaflets of the aortic valve working there.

Up into the ascending aorta.

상승대동맥에는

And it's that part, the ascending aorta,

대동맥 판막이 두개 있는데

바로 그 부분이 팽창되며 터지면

which dilates and ultimately bursts, which of course is fatal.

물론 사망이 초래돼죠.

We started by organizing image acquisition

우리는 우선

from magnetic resonance and CT imaging machines,

환자의 대동맥 모형을

만들기 위해서

from which to make a model of the patient's aorta.

MRI 영상과 CT 이미지를

만들었죠.

이게 제 대동맥 모형입니다.

This is a model of my aorta.

I've got a real one in my pocket,

실제 모형은 제 주머니 안에 있는데

You can see it's quite a complex structure.

보시는 바와 같이 이건 꽤 복잡한 구조인데

It has a funny tri-lobal shape at the bottom,

which contains the aortic valve.

밑 부분에 대동맥 판막이 달려있죠.

It then comes back into a round form,

and then tapers and curves off.

It's quite a difficult structure to produce.

까다로운 구조입니다.

제가 말씀드린데로 이건 제 대동맥의 CAD 모형인데

This is a sort of CAD model of me,

and this is one of the later CAD models.

개발이 많이 된 후의 CAD 모형이죠.

We went through an iterative process of producing better and better models.

우리는 더 낳은 모형을 만들기 위해

반복 과정을 걸쳐 개발 작업을 했죠.

우리는 이 모형을 제작 할 때,

When we produced that model,

we turned it into a solid, plastic model, as you can see,

using a rapid prototyping technique, another engineering technique.

We then used that former

to manufacture a perfectly bespoke porous textile mesh,

which takes the shape of the former and perfectly fits the aorta.

So this is absolutely personalized medicine at its best, really.

Every patient we do has an absolutely bespoke implant.

Once you've made it, the installation is quite easy.

John Pepper, bless his heart, professor of cardiothoracic surgery.

흉부외과 교수인

Never done it before in his life, he put the first one in, didn't like it,

Happy, away I went.

수술대에서 4시간 30분을 보낸 후 수술이 끝났어요.

Four and a half hours on the table, and everything was done.

임플란트를 이식하는 부분은 사실 제일 쉬운 부분이었죠.

So the surgical implantation was actually the easiest part.

우리의 시술 방법과 합성 인공대동맥 접합 수술이라는

If you compare our new treatment to the existing alternative,

기존의 다른 시술방법을 비교하면

the composite aortic root graft,

there are one or two startling comparisons

다음과 같은 한두 가지의

which I'm sure will be clear to all of you.

큰 차이가 있죠

Two hours to install one of our devices,

현재 시술방법은 임플란트를 꼽는데

6시간이 걸리는것에 비해

compared to 6 hours for the existing treatment.

우리 방식은 2시간이면 돼죠.

제가 이미 말씀드린 바와 같이 현재의

As I said, the existing treatment requires the heart-lung bypass machine,

시술 방법은 심폐 우회 장치와

and it requires a total body cooling.

전신냉각이 필요한데 우리 방법은 그런게

We don't need any of that. We work on a beating heart.

전혀 필요없고 심장이 뛰는 상태에서 수술을 하죠.

He opens you up,

외과의는 심장이 뛰는 상태에서 대동맥에 접근하고

he accesses the aorta while your heart is beating,

체온도 정상으로 유지하죠.

all at the right temperature.

No breaking into your circulatory system.

혈액순환계통을 단절할 필요도 없고요.

So it really is great.

그러니까 저희 방법은 정말로 좋죠.

But for me, absolutely the best point is,

하지만 제가 제일 좋아하는 부분은

there is no anticoagulation therapy required.

항응고제를 복용할 필요가 없다는 점이죠.

I don't take any drugs at all,

저는 어떤 약도 먹지 않습니다.

other than recreational ones that I would choose to take.

제가 선택하는 향정신성 물질은 빼고요.

(Laughter)

And in fact, if you speak to people who are on long-term warfarin,

와파린을 장기간 복용한 사람에게 물어보면

it is a serious compromise to your quality of life,

그 약이 삶의 질에 큰 영향을 미친다는 이야기를 하죠.

and even worse, it inevitably foreshortens your life.

그런데 더 나쁜것은

그 약이 수명을 단축시킨다는 거죠.

그리고 인공판막 이식수술을 하면

Likewise, if you have the artificial valve option,

you're committed to antibiotic therapy

침습적인 수술을 받을때 마다

whenever you have any intrusive medical treatment,

반드시 항생제를 먹어야 하고요.

even trips to the dentist require that you take antibiotics,

심지어는 치과 치료를 받더라도 판막이 감염될

in case you get an internal infection on the valve.

가능성이 있기 때문에 항생제를 먹어야 하고요.

Again, I don't have any of that, so I'm entirely free,

저는 그런 위험도 없고 완전히 자유롭죠.

my artery is fixed.

제 대동맥은 이제 고정돼 있기 때문에 터질 걱정을

I haven't got to worry about it, which is a rebirth for me.

할 필요가 전혀 없어서 새로운 삶을 사는 것 같아요.

하여튼, 이 강연의 주제로 다시 돌아오죠.

Back to the theme of the presentation, multidisciplinary research,

보일러 일을 하던 공정 엔지니어가

how on earth does a process engineer used to working with boilers

어떻게 다학문간 연구를 통해

end up producing a medical device which transforms his own life?

자기 자신의 생명을 구하는

의학 장치를 만들게 되었을까요?

Well, the answer to that is, a multidisciplinary team.

그 질문에 대한 답은 다학제팀입니다.

This is a list of the core team,

이건 핵심 팀원들의 목록입니다.

and you can see there aren't only two principal technical disciplines there,

이목록에서

medicine and engineering,

저희팀은 의학과

but also, there are various specialists from within those two disciplines.

John Pepper was the cardiac surgeon who did all the actual work on me.

But everyone else had to contribute one way or another.

Raad Mohiaddin, a medical radiologist.

We had to get good-quality images from which to make the CAD model.

Warren Thornton, who still does all our CAD models for us,

워런 톤튼은 지금도 모든 CAD 모형작업을 해주고 있는데

had to write a bespoke piece of CAD code

to produce this model from this really rather difficult input data set.

There are some barriers to this, though, there are some problems.

그런데 이런 작업을 하는데는 몇가지 장벽과 문제점들이 있죠.

우리가 사용하는 용어가 큰 문제죠.

Jargon is a big one.

I would think no one in this room understands the first four jargon points.

아마 여러분들 중 여기에 있는 첫 4개의 용어를

전부 이해하시는 분은 한명도 없으실 겁니다.

The engineers amongst you will recognize "rapid prototyping" and "CAD."

엔지니어분들은

고속 시제품 제작과 CAD를 아시겠고

The medics amongst you, if there are any, will recognize the first two,

의사분들은 첫 두 단어를 이해하시겠죠.

but there will be nobody else here that understands all those four words.

하지만, 이 단어 4개를

전부 이해하시는 분은 아마 없을 겁니다.

Taking the jargon out was very important

어떤 말을 했을때 팀원 모두가

to ensure that everyone in the team understood exactly what was meant

정확하게 무슨 뜻인지를 이해하게

하려면 전문용어를 사용하지

when a particular phrase was used.

않는 것이 매우 중요했죠.

우리 팀원들이 동의한 용어규약에도 재미있는 일화가 있었죠.

Our disciplinary conventions were funny as well.

We took a lot of horizontal slice images through me,

우리팀은 제 몸의 수평 단면도 이미지를 많이 촬영해서

produced those slices and used them to build a CAD model.

그런 이미지를 CAD 모델을 만드는데 사용했죠.

And the very first CAD model we made,

그런데 우리가 처음으로 플라스틱

the surgeons were playing with it and couldn't quite figure it out.

모델을 만들었을때 외과의들은

그 모델을 이해할 수 없었는데

And then we realized that it was actually a mirror image of the real aorta.

알고 보니까 우리가 만든 모델은 실제 대동맥의

좌우가 바뀐 미러 이미지 모델이었어요.

And it was a mirror image because in the real world,

우리는 건물의 평면도나 지도를 볼때

항상 위에서 아래로 내려 보기 때문에

we always look down on plans,

plans of houses, or streets, or maps.

좌우가 바뀐 미러 이미지의 모델을 만들었던 거죠.

In the medical world, they look up at plans.

의사들은 평면도를 볼때 아래서 위로 올려 보니까

So the horizontal images were all in inversion.

수평 이미지의 좌우가 뒤집혀 질 수 밖에 없었죠.

So, one needs to be careful with disciplinary conventions.

그래서 다학제 팀에서는 규약을 통일하는게 중요하죠.

Everyone needs to understand what is assumed and what is not.

모든 팀원은 어떤 것이 추정되고

어떤것이 추정되지 않았다는 것을 알아야 하죠.

이 프로젝트의 또 다른

Institutional barriers were another serious headache in the project.

문제사항은 제도적인 장벽이었죠.

The Brompton Hospital was taken over

브롬튼 병원은 런던 임페리얼

by the Imperial College School of Medicine.

의대에 의해 인수당했는데

And there are some seriously bad relationship problems

이들간에 심각한

관계상의 문제가 있죠.

between the two organizations.

I was working with the Imperial and the Brompton,

and this generated some serious problems for the project.

Really, problems that shouldn't exist.

Research & Ethics Committee.

If you want to do anything new in surgery,

you have to get a license from your local Research & Ethics.

아마 이곳 폴란드도 마찬가지겠죠.

I'm sure it's the same in Poland.

새로운 종류의 수술 방법을

There will be some form of equivalent which licenses new types of surgery.

인가해 주는 무슨 기관이 있겠죠.

We didn't only have the bureaucratic problems associated with that,

우리는 이런 관료주의적인 장벽 뿐만 아니라,

we also had professional jealousies.

전문가들 간의 좋지않은 경쟁심도 극복해야 했죠.

There were people on the Research & Ethics committee

연구 윤리 위원회의 몇몇 사람들은

who really didn't want to see John Pepper succeed again.

자체를 별로 탐탁하게 생각하지 않았고

Because he is so successful.

And they made extra problems for us.

Bureaucratic problems.

Ultimately, when you have a new treatment,

you have to have a guidance note for all the hospitals in the country.

안내서를 보내야 하죠. 영국에서는

In the UK, we have the National Institute and Clinical Excellence.

NICE라는 국립보건임상연구원이 있는데

폴란드에도 분명히 비슷한게 있을 겁니다.

You have an equivalent in Poland, no doubt.

우리는 NICE의 요구사항을 준수해야 했죠.

And we had to get past the NICE problem.

그래서 우리는 멋진 임상 지침을 인터넷에 올려놨죠.

We now have a great clinical guidance, out on the net.

그래서 관심이 있는 병원은

So any other hospitals interested can come along, read the NICE report,

언제든지 NICE 보고서를 읽고

get in touch with us, and then get doing it themselves.

Funding barriers, another big area to be concerned with.

A big problem with understanding one of those perspectives.

When we first approached one of the big, charitable UK organizations

가장 큰 자선단체중의 하나는 근본적으로

that fund this kind of stuff,

엔지니어링 개발에만 흥미를 가지고 있었고 우리의 계획을

we essentially gave them an engineering proposal.

They didn't understand it, they were doctors, next to God,

it must be rubbish, they binned it.

So in the end, I went after private investors, just gave up on it.

Most R&D is going to be institutionally funded,

by the Polish Academy of Sciences

or the Engineering and Physical Sciences Research Council, or whatever.

And you need to get past those people.

연구개발 지원금을 받았죠.

학제간의 연구를 할때는 용어가 아주 큰 문제이죠.

Jargon is a huge problem when you try to work across disciplines,

왜냐하면 엔지니너들은

because in an engineering world, we all understand CAD and RP.

CAD나 R.P라는 용어를 이해하지만

Not in the medical world.

의사들은 무슨말인지 모르니까요.

저는 연구자금을 관리하는 사람들은 정신을 차려야 한다고 생각합니다.

I suppose the funding bureaucrats ultimately have to get their act together.

그리고 이런걸 기대하는게 지나칠지 모르지만

They've really got to start talking to each other,

그들은 서로 진정한 대화를 하기 시작해야 하고

and exercise a bit of imagination, if that's not too much to ask.

Which it probably is.

I've coined the phrase "obstructive conservatism."

So many people in the medical world don't want to change.

Particularly when some jumped-up engineer has come along with the answer.

They don't want to change.

They simply want to do whatever they've done before.

And in fact, many surgeons in the UK are still waiting

for one of our patients to have some sort of an episode,

We've actually got 30 patients.

At seven and a half years,

we've got 90 post-op patient years between us,

and we haven't had a single problem.

And still, there are people in the UK saying,

"That external aortic root, it will never work, you know."

그거 절대로 안돼요."라고 말하는 사람들이 있죠.

It really is a problem.

여러분들은 언젠가 의사나 외과의사들의

I'm sure everyone in this room has come across arrogance

오만함을 경험하신 적이 있으실 겁니다.

amongst medics, doctors, surgeons, at some point.

The middle point is simply the way that the doctors protect themselves.

슬라이드의 두번째 항목은 간단하게 말해

의사들이 그들 자신을 보호한다는 거죠.

"Well, of course, I'm looking after my patient."

그들은 물론 그들의 환자를 돌봐 준다고 하지만

I think it's not good, but that's my view.

Egos, of course, again a huge problem.

의사의 자부심도 역시 큰 문제죠.

다학제 팀에서 일하면

If you work in a multidisciplinary team,

같이 일하는 동료들을 믿어야하고

you've got to give your guys the benefit of the doubt,

you've got to express support for them.

그들을 지지한다는 것을 말해줘야 하죠.

Incredible guy.

Dead easy to give him respect.

Him giving me respect? Slightly different.

그러나 그가 저를 존경하냐는 약간 다른 얘기죠.

That's all the bad news.

The good news is, the benefits are stonkingly huge.

그런데, 다학제 팀의 장점은 혜택이 아주 막대하다는 거죠.

Translate that one! I bet they can't.

When you have a group of people with different professional training,

전문 교육이 다르고

직업적인 경험이 다른 사람들은

a different professional experience,

they not only have a different knowledge base,

그들의 지식의 분야만 다를 뿐만 아니라

세상을 보는 관점도 다르죠.

but also a different perspective on everything.

그렇지만 그런 사람들을 모아서

And if you can bring them together,

그들이 함께 이야기하고 서로 이해할 수 있게

and get them talking and understanding each other,

the results can be spectacular.

만들면 엄청난 결과가 나올 수 있죠.

You can find really novel solutions that have never been looked at before,

전에는 생각지도 못했던 기발한

해결책을 매우 매우 빨리,

very quickly and easily.

그리고 쉽게 찾을 수 있으니까요.

You can short-cut huge amounts of work

그리고 팀원들의 광범위한 지식기반을 통해 엄청난 량의

simply by using the extended knowledge base you have.

일을 우회할 수 있는 지름길을 찾을 수 있죠.

And as a result, it's an entirely different use

of the technology and the knowledge around you.

The result of all this is that you can get incredibly quick progress

on incredibly small budgets.

I'm so embarrassed at how cheap it was to get from my idea

that I'm not prepared to tell you what it cost,

because I suspect there are absolutely standard surgical treatments,
