It really is a story of Dr. Venkataswamy.

His mission and his message is about the Aravind Eye Care System.

I think first it's important for us to recognize what it is to be blind.

I couldn't thread a needle or see the lice in my hair.

सूई में धागा डालने जैसे काम तो दूर की बात थी, मैं तो अपने बाल की जूँ तक नहीं देख सकती थी ।

If an ant fell into my rice, I couldn't see that either.

अगर मेरे भात (चावल) में चींटी गिर जाती, तो भी मुझे पता नहीं चल सकता था ।

Thulasiraj Ravilla: Becoming blind is a big part of it,

तुलसीराज रविल्ला: नहीं देख पाना तो एक भीषण समस्या है ही,

but I think it also deprives the person of their livelihood, their dignity,

लेकिन अंधापन व्यक्ति से उसका रोज़गार, और यहाँ तक कि उसका स्वाभिमान तक छीन लेता है,

their independence, and their status in the family.

ना उसकी अपनी कोई आज़ादी बचती है और ना ही उसके परिवार में उसका कोई महत्व ।

So she is just one amongst the millions who are blind.

तो आपने देखा कि ये स्त्री उन लाखों लोगों में से एक है जो देख नहीं सकते ।

And the irony is that they don't need to be.

और विडम्बना ये है कि इसे इस दुःख को झेलने की कतई ज़रूरत नहीं है ।

A simple, well-proven surgery can restore sight to millions,

एक साधारण-सी, कई सालों से चली आ रही शल्य-क्रिया से लाखों लोगों की दृष्टि वापस आ सकती है ।

and something even simpler, a pair of glasses, can make millions more see.

और उससे भी साधारण तरीका, एक चश्मा, जाने कितने ही और लोगों को देखने लायक बना सकता है ।

If we add to that the many of us here now

अगर यहाँ बैठे लोगों के स्तर पर बात की जाय

who are more productive because they have a pair of glasses,

जो कि चश्मे के कारण कार्य-कुशल है,

then almost one in five Indians will require eye care,

तो लगभग हर पाँच में से एक भारतीय को नेत्र-चिकित्सा की आवश्यकता है,

a staggering 200 million people.

Today, we're reaching not even 10 percent of them.

मौजूदा स्थिति ये है कि हम इनमें से १० प्रतिशत तक भी नहीं पहुँच सके हैं ।

So this is the context in which Aravind came into existence

अरविन्द की कहानी का यही संदर्भ है

about 30 years back

करीब ३० साल पहले

as a post-retirement project of Dr. V.

डॉ. वी ने रिटायरमेंट (अवकाश-प्राप्ति) के बाद एक बीडा उठाया ।

He started this with no money.

उन्होनें बिना किसी पूँजी के शुरुवात की ।

He had to mortgage all his life savings

उन्हें अपने जीवन की सारी बचत, सारी संपत्ति गिरवी रखनी पडी थी

to make a bank loan.

तब जा कर बैंक से लोन मिल पाया था ।

And over time, we have grown into a network of five hospitals,

और धीरे-धीरे, हम पाँच अस्पतालों के समूह में विकसित हो गये,

predominately in the state of Tamil Nadu and Puducherry,

ज्यादातर, तमिलनाडु और पुड्डुचेरी में,

and then we added several, what we call Vision Centers

और फिर, हमने विज़न सेंटरों को विकसित करना शुरु किया

as a hub-and-spoke model.

And then more recently we started managing hospitals

in other parts of the country

and also setting up hospitals in other parts of the world as well.

The last three decades,

we have done about three-and-a-half million surgeries,

a vast majority of them for the poor people.

Now, each year we perform about 300,000 surgeries.

आज हम हर साल लगभग ३ लाख ऑपरेशन करते हैं ।

A typical day at Aravind, we would do about a thousand surgeries,

किसी भी दिन औसतन हम लोग अरविन्द में एक हज़ार ऑपरेशन करते हैं ।

maybe see about 6,000 patients,

और लगभग ६ हज़ार मरीज़ देखते हैं,

send out teams into the villages to examine, bring back patients,

हम अपनी टीमों को गाँव-गाँव भेजते है, वहाँ से ज़रूरतमंद मरीज़ों को लाते हैं,

lots of telemedicine consultations,

कई बार दूर-संचार के माध्यम से भी इलाज करते हैं,

and, on top of that, do a lot of training,

और इस सब को कर पाने के लिये, बडे पैमाने पर प्रशिक्षण भी देते हैं ।

both for doctors and technicians who will become the future staff of Aravind.

उन डाक्टरों और तकनीशियनों को, जो कि आगे चल कर अरविन्द के कार्यकर्ता बनते हैं ।

And then doing this day-in and day-out, and doing it well,

और फिर, हम यही काम दिन-रात, रात-दिन, बारम्बार करते जाते हैं, और बेहतर करते जाते हैं,

requires a lot of inspiration and a lot of hard work.

And I think this was possible thanks to the building blocks

put in place by Dr. V.,

a value system, an efficient delivery process,

Dr. V: I used to sit with the ordinary village man because I am from a village,

and suddenly you turn around and seem to be in contact with his inner being,

और अचानक ऐसा लगता है जैसे आप इस व्यक्ति की अंतरात्मा से जुड रहे हों,

you seem to be one with him.

आप उसके साथ मिल कर एक हो रहे हों।

Here is a soul which has got all the simplicity of confidence.

उस आम आदमी की आत्मा में विश्वास की सादगी भरी होती है ।

Doctor, whatever you say, I accept it.

डाक्टर, जो भी आप कहेंगे, मैं करने को तैयार हूँ ।

An implicit faith in you

मैं आप में अपना पूर्ण विश्वास रखता हूँ

and then you respond to it.

और आप उस विश्वास को किसी भी हालत में तोड नहीं सकते ।

Here is an old lady who has got so much faith in me, I must do my best for her.

मेरे सामने एक बूढी औरत है जिसे मुझमें संपूर्ण विश्वास है, मेरा कर्तव्य है कि मैं पूरी कोशिश करूँ ।

When we grow in spiritual consciousness,

जब हम आत्मा को चेतन करते हैं,

we identify ourselves with all that is in the world,

हम सारे संसार को स्वयं का हिस्सा समझने लगते हैं,

so there is no exploitation.

इसलिये लेन-देन का सवाल ही नहीं उठता ।

It is ourselves we are helping.

हम तो स्वयं अपनी ही मदद कर रहे होते हैं ।

It is ourselves we are healing.

हम तो स्वयं अपना ही इलाज कर रहे होते हैं ।

This helped us build a very ethical and very highly patient-centric organization

इन विचारधारा से प्रेरित होकर हमें पूर्णतः रोगी-केंद्रित और आदर्श संस्था बनाई

and systems that support it.

But on a practical level, you also have to deliver services efficiently,

ऐसी संस्था चलाने के लिये आपको सर्वोत्तम सेवा सुचारु रूप से देनी पडती है,

and, odd as it may seem, the inspiration came from McDonald's.

और अजीब लगेगा पर मुझे इसकी प्रेरणा मक्डोनॉल्डस्‌ से मिली ।

Dr. V: See, McDonald's' concept is simple.

डॉ. वी: देखिये, मक्डोनॉल्डस् का सिद्धान्त साधारण सा है ।

They feel they can train people all over the world,

उन्हें लगता है कि वो दुनिया भर के लोगों को प्रशिक्षण दे सकते हैं,

irrespective of different religions, cultures, all those things,

अलग-अलग धर्मों, संस्कृतियों वगैरह वगैरह के बावजूद,

to produce a product in the same way

and deliver it in the same manner

और उसी ख़ास तरह से प्रस्तुत करने का

in hundreds of places.

Larry Brilliant: He kept talking about McDonalds and hamburgers,

लैरी ब्रिलियन्ट: वो हमेशा मक्डोनल्ड और हैम-बर्गर की बात करते रहते थे,

and none of it made any sense to us.

और हमें कुछ भी समझ नहीं आता था ।

He wanted to create a franchise,

वो उप-संस्थानो का निर्माण करना चाहते थे,

a mechanism of delivery of eye care

with the efficiency of McDonald's.

Dr. V: Supposing I'm able to produce eye care,

डॉ. वी.: मान लीजिये मैं आँखों के इलाज का एक अदद तरीका निकाल लूँ,

techniques, methods, all in the same way,

सारी तकनीकें, सारी कार्यप्रणालियाँ, बिलकुल एक सी,

and make it available in every corner of the world.

और उसे बंद-डिब्बा रूप में दुनिया के हर कोने में पहुँचा सकूँ ।

The problem of blindness is gone.

अँधेपन की समस्या हो गयी गायब, चुटकी बजाते ही ।

TR: If you think about it, I think the eyeball is the same,

टी.आर. : यदि आप बारीकी से सोचें, तो आँखों की पुतली एक सी ही होती है,

as American or African,

चाहे अमरीकन हो या फिर अफ़्रीकन,

the problem is the same, the treatment is the same.

समस्या भी एक सी है, इलाज भी एक सा हो सकता है ।

And yet, why should there be so much variation in quality and in service,

तो फिरे कोई वजह नहीं कि गुणवत्ता और सेवा में इतना ज्यादा फर्क हो,

and that was the fundamental principle that we followed

और हमने यही मूल सिद्धान्त अपनाया

when we designed the delivery systems.

जब हमने सेवाओं पद्धति का विकास किया ।

And, of course, the challenge was that it's a huge problem,

और सच है कि ये एक बहुत विशाल चुनौती थी,

we are talking of millions of people,

very little resource to deal with it,

and then lots of logistics and affordability issues.

And then so, one had to constantly innovate.

And one of the early innovations, which still continues,

is to create ownership in the community to the problem,

and then engage with them as a partner,

and here is one such event.

Here a community camp just organized

by the community themselves,

where they find a place, organize volunteers,

and then we'll do our part. You know, check their vision,

and then you have doctors who you find out what the problem is

and then determine what further testing should be done,

and then those tests are done by technicians

who check for glasses,

or check for glaucoma.

And then, with all these results, the doctor makes a final diagnosis,

और अंततः, सारी जाँचों के नतीज़े के आधार पर डॉक्टर निर्णय ले कर

and then prescribes a line of treatment,

इलाज का निर्णायक तरीका सुझाते हैं,

and if they need a pair of glasses, they are available right there at the camp site,

यदि किसी को चश्मे की ज़रूरत है, तो उसे शिविर में ही तुरंत चश्मा दिया जाता है,

usually under a tree.

But they get glasses in the frames of their choice,

and that's very important because I think glasses,

in addition to helping people see,

is also a fashion statement, and they're willing to pay for it.

So they get it in about 20 minutes

and those who require surgery, are counseled,

and then there are buses waiting,

which will transport them to the base hospital.

And if it was not for this kind of logistics and support,

खरी बात ये है कि यदि इस हद तक सेवा उपलब्ध न हो,

many people like this would probably never get services,

तो इनमें से बहुत से लोगों कभी भी चिकित्सा नहीं पा पायेंगे,

and certainly not when they most need it.

सही वक्त पर तो बिलकुल भी नहीं ।

They receive surgery the following day,

शल्य चिकित्सा भी सलाह देने के ठीक दूसरे दिन ही दी जाती है,

and then they will stay for a day or two,

उसके बाद उन्हें एक दो दिन तक देखभाल के लिये रोका जाता है,

and then they are put back on the buses

फिर उन्हीं बसों से वापस भेज दिया जाता है,

to be taken back to where they came from,

जहाँ से वो आये थे,

and where their families will be waiting to take them back home.

और जहाँ उनके परिवारजन बेसब्री से उन्हें वापस ले जाने के इन्तज़ार में होते हैं ।

And this happens several thousand times each year.

और ये पूरी प्रक्रिया साल मे कई हज़ार बाद होती है ।

It may sound impressive that we're seeing lots of patients,

आप सब हर्षित होंगे कि हम बहुत रोगियों का इलाज कर रहें हैं,

very efficient process,

हमारी कार्यप्रणाली अत्यंत सुचारु है,

but we looked at, are we solving the problem?

पर हमने सोचा कि क्या हम वास्तव में समस्या हल कर पा रहे हैं ?

We did a study, a scientifically designed process,

हमने एक अध्ययन किया - वैज्ञानिक पद्धति से,

and then, to our dismay,

और हमें बहुत आश्चर्य हुआ जब

we found this was only reaching seven percent of those in need,

and we're not adequately addressing more, bigger problems.

So we had to do something different,

so we set up what we call primary eye care centers, vision centers.

These are truly paperless offices

with completely electronic medical records and so on.

They receive comprehensive eye exams.

We kind of changed the simple digital camera into a retinal camera,

and then every patient gets their teleconsultation with a doctor.

इस तरह हर रोगी को नेत्र-विशेषज्ञ से सीधे जोडा जा सका।

The effect of this has been that, within the first year,

इसका असर ये हुआ, कि पहले ही साल में,

we really had a 40 percent penetration in the market that it served,

हम सात से चालीस प्रतिशत ज़रूरतमंद लोगों तक पहुँच पाने में सक्षम हो गये,

which is over 50,000 people.

जो कि पचास हज़ार लोगों से भी ज्यादा का समुदाय है।

And the second year went up to 75 percent.

और दूसरे साल में, हम पचहत्तर प्रतिशत तक पहुँच गये ।

So I think we have a process by which

we can really penetrate into the market

and reach everyone who needs it,

हर एक ज़रूरतमंद तक पहुँच सकते है,

and in this process of using technology, make sure

यही नहीं, इस तकनीक के प्रयोग से हम ये भी सुनिश्चित करते हैं

that most don't need to come to the base hospital.

कि ज्यादा से ज्यादा लोगों को अस्पताल आने के ज़रूरत न पडे ।

And how much will they pay for this?

और इसके लिये उन्हें कितने पैसे देने होंगे ?

We fixed the pricing, taking into account

हमने एक दाम पक्का किया, ये देखा

what they would save in bus fare in coming to a city,

कि वो शहर ना आने से कितने पैसे बचा रहे है,

so they pay about 20 rupees, and that's good for three consultations.

तो वो हमें देते हैं कुल बीस रुपये, और इसके बदले में तीन बार डॉक्टरों से मिल सकते हैं ।

The other challenge was, how do you give high-tech

एक और चुनौती थी, कि कैसे उच्च-तकनीकी

or more advanced treatment and care?

We designed a van with a VSAT,

which sends out images of patients to the base hospital

where it is diagnosed,

and then as the patient is waiting, the report goes back to the patient,

और रोगी के देखते ही देखते, उसकी रिपोर्ट भी वापस वैन तक आ जाती है,

it gets printed out, the patient gets it,

प्रिंट हो जाती है, और रोगी को मिल भी जाती है,

and then gets a consultation about what they should be doing --

और विशेषज्ञ की सटीक सलाह उन्हें मिलती है,

I mean, go see a doctor or come back after six months,

यानि, अभी तुरंत डॉक्टर से मिलिये, या फिर छः महीने बाद जाँच कराइये,

and then this happens as a way of

bridging the technology competence.

को आम जनता तक पहुँचा कर ।

So the impact of all this has been essentially one of growing the market,

इसक एक असर ये भी है कि जरूरतमंदों की संख्या भी बढी है,

because it focused on the non-customer,

इस तकनीक ने कई लोगों को चिकित्सा की आवश्यकता के प्रति जागरुक किया है,

and then by reaching the unreached,

और यही नहीं, चिकित्सा को उन तक पहुँचाया भी है,

we're able to significantly grow the market.

The other aspect is how do you deal with this efficiently

इस सबका दूसरा पहलू ये है कि ये सब सुचारु रूप से कैसे चलाया जाय

when you have very few ophthalmologists?

जबकि आपके पास केवल मुट्ठी भर नेत्र-विशेषज्ञ हैं ?

So what is in this video is a surgeon operating,

and then you see on the other side,

another patient is getting ready.

So, as they finish the surgery,

they just swing the microscope over,

the tables are placed so that their distance is just right,

and then we need to do this, because, by doing this kind of process,

we're able to more than quadruple the productivity of the surgeon.

And then to support the surgeon,

we require a certain workforce.

And then we focused on village girls that we recruited,

and then they really are the backbone of the organization.

They do almost all of the skill-based routine tasks.

They do one thing at a time. They do it extremely well.

With the result we have very high productivity,

very high quality at very, very low cost.

So, putting all this together, what really happened was

significantly higher than anyone else.

As a result, our complications are significantly lower

So the final part of the puzzle is,

how do you make all this work financially,

especially when the people can't pay for it?

So what we did was, we gave away a lot of it for free,

and then those who pay, I mean, they paid local market rates,

nothing more, and often much less.

And we were helped by the market inefficiency.

और बाज़ार कभी भी हमसे उत्पादक्ता में मुकाबला नहीं कर पाये ।

I think that has been a big savior, even now.

शायद हमारी सफ़लता की कुंची यही है, आज भी ।

And, of course, one needs the mindset to be wanting to give away

असल में, आपके पास जो फालतू पडा है, उसे बाँटने के लिये

what you have as a surplus.

एक खास मानसिकता की ज़रूरत होती है ।

The result has been, over the years,

परिणाम ये है कि साल दर साल,

the expenditure has increased with volumes.

जहाँ हमारा खर्च दिन दूना रात चौगुना बढा है,

The revenues increase at a higher level,

वहीं हमारी आमदनी उससे भी ज्यादा रफ़तार से बढी है,

giving us a healthy margin

जिससे हमें एक अच्छा मुआवज़ा मिला है

while you're treating a large number of people for free.

जबकि हम तमाम लोगों का मुफ़्त इलाज़ भी कर रहे हैं ।

I think in absolute terms,

मैं मोटा मोटा हिसाब बताता हूँ,

last year we earned about 20-odd million dollars,

पिछ्ले साल, हमने करीब दो करोड डॉलर की आमदनी की,

spent about 13 million, with over a 40 percent EBITA.

और खर्च करीब डेढ करोड का, मतलब चालीस प्रतिशत मुनाफ़ा

But this really requires going beyond what we do,

if you really want to achieve solving this problem of blindness.

And what we did was a couple of very counter-intuitive things.

We created competition for ourselves,

and then we made eye care affordable

by making low-cost consumables.

We proactively and systematically

हमने ज़ोर-शोर से पर एक व्यवस्थित अंदाज में

promoted these practices to many hospitals in India,

इन तकनीकों को भारत के कई एक अस्पतालों में लागू किया

many in our own backyards and then in other parts of the world as well.

और भारत के बाहर भी ।

The impact of this has been that these hospitals,

तकनीक को सबके साथ बाँटने से हुआ ये कि

in the second year after our consultation,

हमारे साथ काम करने के दूसरे ही साल में,

are double their output

अस्पतालों की क्षमता दुगुनी हो गयी,

and then achieve financial recovery as well.

और आर्थिक रूप से भी वो सक्षम हो गये ।

The other part was how do you address

this increase in cost of technology?

There was a time when we failed to negotiate

एक समय था कि हम लाख चाहने पर भी

the [intra-ocular lens] prices to be at affordable levels,

कृत्रिम लेंस को कम कीमत पर उपलब्ध नहीं करवा पाये,

so we set up a manufacturing unit.

तो हमने उन्हें खुद ही बनाने की ठान ली ।

And then, over time, we were able to bring down the cost significantly

to about two percent of what it used to be when we started out.

Today, we believe we have about seven percent of the global market,

and they're used in about 120-odd countries.

To conclude, I mean, what we do, does it have a broader relevance,

or is it just India or developing countries?

So to address this, we studied UK versus Aravind.

तो हमने अरविन्द और यूके का एक तुलनात्मक अध्ययन किया ।

What it shows is that we do roughly about 60 percent

हमें ये पता लगा कि हम यूके का लगभग साठ प्रतिशत

of the volume of what the UK does,

काम अकेले ही कर लेते हैं,

near a half-million surgeries as a whole country.

पूरा यूके लगभग पाँच लाख शल्य-क्रियाएँ करता है ।

And we do about 300,000.

और हम करीब तीन लाख ।

And then we train about 50 ophthalmologists

इसके ऊपर हम हर साल करीब पचास नेत्र-विशेषज्ञों को प्रशिक्षित भी करते हैं

against the 70 trained by them,

जबकि पूरे यूके में सत्तर ऐसे विशेषज्ञ प्रशिक्षित होते हैं,

comparable quality, both in training and in patient care.

So we're really comparing apples to apples.

We looked at cost.

So, I think it is simple to say

just because the U.K. isn't India the difference is happening.

I mean, I think one has to look at other aspects as well.

Maybe there is --

the solution to the cost could be in productivity,

maybe in efficiency, in the clinical process,

or in how much they pay for the lenses or consumables,

या फिर रोगी लेंस के लिये या और सुविधाओं के लिये कितना दाम दे रहे हैं,

or regulations, their defensive practice.

या फिर, बेहतर कानूनों मे, या रक्षात्मक कार्यप्रणालियों में

So, I think decoding this can probably bring

इस समस्या को यदि सुलझा लिया जाय तो

answers to most developed countries

विकसित देशों को भी कई हल मिल सकते हैं

including the U.S., and maybe

यू.एस, और

Obama's ratings can go up again.

शायद ओबामा को लोग दोबारा चाहने लगें ।

Another insight, which, again, I want to leave with you,

in conditions where the problem is very large,

which cuts across all economic strata,

और सारे आर्थिक स्तरों के लोग उससे जूझ रहे हों,

where we have a good solution,

तो एक हल मिल सकता है,

I think the process I described,

मेरे ख्याल से जो पद्धति मैनें बताई है,

you know, productivity, quality, patient-centered care,

जैसे कि, उत्पादक्ता, गुणवत्ता, रोगी-केन्द्रित सेवा,

can give an answer,

वो इसका हल बन सकती है,

and there are many which fit this paradigm.

और कई समस्याएँ हैं जिन पर ये पद्धति लागू हो सकती है ।

You take dentistry, hearing aid, maternity and so on.

दँत चिकित्सा या श्रवण समस्या, या फिर जच्चा-बच्चा इत्यादि

There are many where this paradigm can now play,

अनेकों समस्याओं का हल इस पद्धति में छुपा बैठा है,

but I think probably one of the most challenging things

पर मुझे लगता है कि इस पद्धति की सबसे बडी कठिनाई है

is on the softer side.

भावनाओं से जुडी हुई

Now, how do you create compassion?

आप करुणा की भावना कैसे पैदा करेंगे ?

Now, how do you make people own the problem,

कैसे आप लोगों से दूसरों की समस्याओं को अपनाने की बात कहेंगे,

want to do something about it?

कैसे आप उन्हें कुछ करने के लिये प्रेरित करेंगे ?

There are a bit harder issues.

ये बहुत गहन मसले हैं ।

And I'm sure people in this crowd can probably find the solutions to these.

और मुझे विश्वास है कि यहाँ बैठे लोगों में हल निकालने की काबलियत है ।

So I want to end my talk leaving this thought and challenge to you.
