Human beings are the only animals capable of contemplating

When she was in her mid-20s, in the 1920s,

my grandmother was already deeply familiar with the sequence of events

And that’s because, as a woman --

and it was usually women’s work --

she was doing what women had done for centuries,

And yet, when I reached my mid-twenties,

in the 1980s,

I had none of her wisdom and understanding and knowledge of dying,

I'd spent five years being trained to stop people from dying.

Why the difference,

Think about what happened to medicine over the course of the 20th century.

It was not worth going to hospital when you were dying

But by the 1960s, '70s, '80s and onwards,

there were really clever anesthetics

that allowed surgeons to spend a long time unpicking things

There were new and very sophisticated treatments for cancers,

Medical progress was astonishing,

and that is fantastic.

By taking dying people out of home and putting them in hospital,

we changed our understanding of the process.

We lost our ownership of the process and we gave it to healthcare ...

And we forgot what dying looked like.

So, having been qualified for just over four years,

I find myself in a new job.

Having originally intended a career in cancer medicine,

I'd learned a lot of medicine,

I'd seen a lot of dying.

We had a patient in the hospice.

She was a memorable woman for many reasons.

In fact, she was a little bit scary.

One day, she told the nurse who was looking after her

that she was terrified of dying in agony,

because if she were to die in agony, she might despair in God.

And if she were to despair in God,

as a French Roman Catholic,

her belief was that that would be a mortal sin,

so she would not be able to go to heaven,

and heaven was the place she knew her husband was waiting for her.

This was a profound existential distress.

And my boss said, "We need to go and talk to her, and you should come,

I was 26.

It's the last age when you know that you know everything.

So I went along, wondering what I might learn,

because I thought I was quite good at pain control.

And it's brought me here.

Sitting on her bed, with me on a little footstool

so I can see him and her, and the nurse sitting on the chair,

he said to her,

"I'm concerned that you've got worries about what might happen as you're dying."

And she said yes. She knew him well, she trusted him.

And he said, "I'm sorry to hear that,

And I'm sitting on the stool of all knowledge, thinking,

"You can't tell her that."

(Laughter)

Because I’ve seen lots of dying, and I know they’re all different.

And she said, "Yes, please."

And he said, "I'll describe what we usually see,

The thing that's really interesting, Sabine,

is that as people are dying,

it doesn't really matter what the illness is they're dying from.

The pattern of events is very similar.

We see people becoming more and more tired.

It's harder and harder for them to find the energy to do things.

In fact, they recharge their energy not so much by eating and drinking,

but by sleeping.

And as time goes by,

what we see is that people sleep more and they're awake less.

And if they want to do something important,

သူတို့ အရေးကြီးတာတစ်ခုခုကို လုပ်ချင်ရင်၊

they should take a snooze before it."

She nodded, and she got hold of his hand.

we need to waken them ...

For a period, we can't waken them.

They're not just asleep,

အိပ်ပျော်ရုံသာမဟုတ်ဘဲ၊

they're actually unconscious.

တကယ်တော့ သတိလစ်နေကြတာပါ။

And when they waken, they tell us they've had a lovely sleep.

It turns out that human beings don't recognize

သတိလစ်သွားတဲ့အချိန်ကို လူသားတွေဟာ

when we become unconscious.

မမှတ်မိဘူးဆိုတာပါ။

And so at the very end of somebody's life, they're not just asleep,

they're actually deeply unconscious.

And when the brain is unconscious,

the only part of it that's still working

is the part that's working their breathing."

By now, she is sitting right up in bed.

အပိုင်းက အသက်ရှူတာကို လှုံ့ဆော်တဲ့ အပိုင်းပဲ”တဲ့။

And so breathing cycles we don't normally see start to happen.

ဒီတော့ အသက်ရှူ သံသရာတွေက ပုံမှန်တော့ စတင်ဖြစ်လာတာကို မမြင်ရဘူး။

Reflex automatic breathing

နက်ရှိုင်းတဲ့ အသက်ရှူသံသရာတွေကနေ အမှတ်မဲ့

cycles from very deep breaths becoming shallower and shallower,

အလိုအလျောက် အသက်ရှူ သံသရာတွေက ပိုပြီးညင်းညင်းလာတယ်။

and then going back to the beginning again,

ပြီးတော့ အစကို ပြန်သွားပြီး

cycles of fast breathing that gradually become slower,

မြန်ဆန်တဲ့ အသက်ရှူ သံသရာတွေဟာ တဖြည်းဖြည်း နှေးလာတယ်၊

maybe with pauses,

ရပ်တန့်မှုတွေပါကောင်းပါမယ်၊

and then back to the beginning again.

ပြီးတော့ အစကို ပြန်သွားပြန်တယ်။

The person can't feel their throat.

ဒီလူက သူ့လည်ချောင်းကို မခံစားနိုင်ဘူး။

They don't notice if they breathe out through their voice box,

သူ့ရဲ့ အသံအိုးကနေ အသက်ရှူလား ဆိုတာတောင် သတိမပြုမိဘူး၊

making a noise families might think that they're sighing

ဆူညံရင်း မိသားစုတွေက သူ သက်ပြင်းချနေတယ်၊ ညည်းတွားနေတယ်၊ မသက်မသက်သာ

or groaning, or uncomfortable.

ဖြစ်နေတယ်လို့ တွေးလောက်တယ်

We'll always check,

ကျွန်မတို့ အမြဲစစ်ဆေးပေမဲ့၊

but it's part of this reflex breathing.

ဒါက ဒီအမှတ်မဲ့ အသက်ရှူတာရဲ့ တစ်စိတ်တစ်ပိုင်းပါ။

Saliva or mouth-cleaning fluid won't irritate their throat,

သွားရည် ဒါမဟုတ် ပါးစပ် သန့်စင်ရေး အရည်က လည်ချောင်းကို အနှောင့်ယှက်မှာမဟုတ်ဘူး။

it won't make them cough or swallow.

ဒါက သူကို ချောင်းဆိုး၊ မျိုချစေမှာ မဟုတ်ဘူး။

They just lie there with a little pool of fluids,

တစ်ခါတလေ လည်ချောင်းနောက်က အရည်အိုင်လေးတစ်ခုနဲ့

sometimes in the back of their throat.

လဲလျောင်းကြတယ်။

It's not in the way.

အဟန့်အတားတော့ဖြေစ်ဘူး။

Air is moving in and out of their lungs,

လေက သူ့ရဲ့အဆုတ်ထဲကနေ ဝင်၊ထွက်နေတယ်၊

and it bubbles through that little film of fluid,

ဒါကအရည်အလွှာပါးလေးကနေ ပလုံစီပေမဲ့၊

but families can mistake that for drowning or choking.

မိသားစုတွေက ရေနစ်တာ ဒါမှမဟုတ် ရေမွန်းတာလို့ အထင်မှားနိုင်တယ်။

So one of the things that we'll do, Sabine,

ကျွန်မတို့ လုပ်မယ့် တစ်ခုက၊ "Sabine ရေ၊

if your nieces and nephews are here,

တူတွေနဲ့ တူမတွေ ဒီမှာရှိရင်၊ ဖြစ်နေတာကို

is we will make sure we explain to them what is happening to you."

သူတို့ကို ရှင်းပြပေးရမယ်ဆိုတာ သေချာအောင်လုပ်နော်"ဆိုတာပါ။

She's stroking his hands, she's nodding.

လူနာက သူ့လက်ကို ပွတ်သပ်ရင်း၊ ခေါင်းညိတ်နေတယ်။

She is absolutely taking in everything that he says.

လူနာက သူ ပြောသမျှကို လုံးဝ နာယူနေတာပါ။

And then he says -- and I think,

ပြီးတော့ သူ ပြောတာက "အိုး ဘုရားရေ၊

"Oh my goodness, he's going to the last breath."

နောက်ဆုံး ထွက်သက်ဖြစ်တော့မယ်" လို့ ကျွန်မ ထင်တယ်။

"And then, during, usually, one of those phases of slow breathing,

ဒီနောက် ဒီအချိန်အတွင်း၊ အသက် ဖြည်းဖြည်း ရှူတာရဲ့ အဆင့်တွေထဲက တစ်ခုက

there'll be a breath out ...

အသက်ရှူထုတ်တာပါ...

that just isn't followed by another breath in.

နောက်ထပ် အသက်ရှူတာ မရှိတော့ပါဘူး။

There is nothing special about the last breath.

နောက်ဆုံး ထွက်သက်နဲ့ပတ်သက်ပြီး အထူးအဆန်း ဘာမှမရှိဘူး။

It's so not like on the television or in cinema.

ဒါက ရုပ်မြင်သံကြားမှာ ဒါမှမဟုတ် ရုပ်ရှင်မှာလို မဟုတ်ဘူး။

There's no rush of pain at the end. There's no sudden panic.

အဆုံးမှာ ရုတ်တရက် နာကျင်တာ မရှိပါဘူး။ ရုတ်တရက် ထိတ်လန့်တာ မရှိဘူး။

There's no feeling of fading away.

ပျောက်ကွယ်သွားတဲ့ ခံစားချက် မရှိဘူး။

Sometimes, we who work in palliative care,"

တစ်ခါတလ ရောဂါသက်သာရေး စောင့်ရှောက်မှုမှာ အလုပ်လုပ်တော့

and I've subsequently discovered this to be true,

ဒါက မှန်နေတာကို နောက်ပိုင်းမှာ ရှာဖွေတွေ့ရှိခဲ့တယ်။

"sometimes, we will walk into a room where a family has been around a person

တစ်ခါတလေ၊ သေဆုံးတော့မယ့် လူရဲ့အနားမှာ မိသားစုတစ်စုရှိနေတဲ့ အခန်းတစ်ခန်းထဲကို

who is in the act of dying,

ဝင်သွားရမှာဖြစ်ပြီး၊

and we'll realize the person has stopped breathing,

ဒီလူက အသက်ရှူ ရပ်သွားတာကို ကျွန်မတို့ သိမှာဖြစ်ပေမဲ့၊

and the family hasn't noticed yet,

မိသားစုက သတိမထားမိသေးတာက၊

because the Hollywood finale that they're waiting for hasn't happened."

သူတို့ စောင့်နေတဲ့ ဟောလိဝုဒ် ဇာတ်သိမ်းခန်း ဖြစ်မလာသေးလို့ပါ။

She got hold of both of his hands.

လူနာက သူ့လက်နှစ်ဖက်စလုံးကို ဆုပ်ကိုင်ထားတယ်။

She shook them in hers,

လူနာက ဆရာ့လက်တွေကို သူ့လက်ထဲမှာ လှုပ်တယ်၊

and then she pulled his hands to her face and she kissed them.

ဒီနောက် သူ့လက်တွေ သူ့မျက်နှာဆီ ဆွဲတင်လိုက်ပြီး နမ်းလိုက်တယ်။

And then, she closed her eyes,

ပြီးတော့ လူနာက မျက်လုံးတွေ မှိတ်လိုက်ပြီး

she lay back on her pillows.

ခေါင်းအုံးတွေပေါ် ပြန်လဲလိုက်တယ်။

I just watched her relax,

သူ့ရဲ့ အေးဆေးငြိမ်သက်မှုကို ကျွန်မ ကြည့်လိုက်တယ်၊

and in her own inimitable and aloof way,

သူ့ကိုယ်ပိုင် တုပလို့မရတဲ့၊ အဖက်မလုပ်တဲ့ဟန်နဲ့

she told us that we were no longer required.

ကျွန်မတို့ကို မလိုအပ်တော့ဘူးလို့ ပြောတယ်။

(Laughter)

And my boss said to me, “Are you OK?” And I said, “Yes.” (Voice breaking)

အကြီးအကဲက "အဆင်ပြေလား" တဲ့။ "ရတယ်" လို့ ပြောလိုက်တယ်။(ဆို့နင့်သံ)

And I went to the kitchen to blow my nose and dry my eyes,

ပြီးတော့ နှပ်ညှစ်ပြီး မျက်ရည်သုတ်ဖို့ ကျွန်မ မီးဖိုချောင်ကို သွားပြီး

and think about what just happened.

ဖြစ်ပျက်ခဲ့တာတွေ စဉ်းစားမိတယ်။

Two huge ideas exploding in my brain at the same time.

တစ်ချိန်တည်း ကျွန်မရဲ့ ဦးနှောက်ထဲမှာ ကြီးမားတဲ့ စိတ်ကူးနှစ်ခုက ပေါက်ကွဲနေတယ်။

One is "How have I never noticed that?"

တစ်ခုကတော့ "ဒါကို ငါ ဘယ်တုန်းက သတိမထားပါလား" ဆိုတာပါ။

That pattern that he has just explained,

အကြီးအကဲက အခုလေးတင် ရှင်းပြတဲ့ အဲဒီပုံစံကို

I have seen hundreds of times.

ကျွန်မ အကြိမ်ပေါင်း ရာနဲ့ချီ မြင်ခဲ့ တယ်။

But I was the most junior doctor.

ဒါပေမဲ့ ကျွန်မက အငယ်ဆုံး ဆရာဝန်ပါ။

It was my job to stop the person dying, remember?

လူတွေ သေတာကို တားဆီးပေးဖို့က ကျွန်မရဲ့အလုပ်လေ၊ မှတ်မိသေးလား။

So I was so busy worrying about this person's oxygen levels

ဒီတော့ ဒီလူရဲ့ အောက်ဆီဂျင် ပမာဏနဲ့ ဒီလူရဲ့ သွေးခုန်နှုန်း၊

and that person's pulse, and this person's kidney function

ကျွန်မ နောက်မဆုတ်တဲ့ ဒီလူရဲ့ ကျောက်ကပ် လုပ်ဆောင်မှုနဲ့ပတ်သက်ပြီး

that I didn't stand back and see that there's a pattern going on here.

ကျွန်မ ပူပန်ရင်း အလုပ်ရှုပ်နေတယ်၊ ဒီမှာ ဆက်ဖြစ်နေတဲ့ ပုံစံတစ်ခုကို မြင်တယ်။

We can describe the process of ordinary human dying,

ကျွန်မတို့ဟာ သာမန် လူသား သေဆုံးတဲ့ ဖြစ်စဉ်ကို ဖော်ပြနိုင်ပါတယ်။

and it's as much a process as the process of giving birth is.

ဒါဟာ ကလေးမွေးတဲ့ လုပ်ငန်းစဉ်လောက် များပြားတဲ့ ဖြစ်စဉ်တစ်ခုပါ။

It has stages.

အဆင့်တွေရှိပါတယ်။

We can recognize them, we can pace ourselves.

ဒါတွေကို ကျွန်မတို့ မှတ်မိနိုင်တယ်၊ အရှိန်မြှင့်နိုင်တယ်။

We can work out where we are in it.

ကျွန်မတို့ ဘယ်မှာ ရှိတာကို အဖြေရှာနိုင်တယ်။

But even more fascinating

ဒါပေမဲ့ ပိုတောင်စိတ်ဝင်စားစရာက

was that realization from watching Sabine's reaction,

Sabine ရဲ့ တုံ့ပြန်မှုကို ကြည့်တာကနေ သာမန် သေဆုံးတာကို

that we can describe ordinary dying to a dying person ...

သေတော့မယ့် လူတစ်ယောက်ကို ပြောပြနိုင်တဲ့ အသိတရားပါ။

and it shines the light of understanding and information

ဒါက သူတို့ရဲ့ ကြောက်ရွံ့မှုတွေနဲ့ စိတ်ကူးတွေ အားလုံး ပါဝင်နေတဲ့

into that dark place

မှောင်မိုက်တဲ့ နေရာထဲမှာ နားလည်မှုနဲ့

where all their fears and imagination were at play.

သတင်းအချက်အလက်တွေရဲ့ အလင်းကို ထွန်းလင်းပေးတယ်။

I've gone on in my career in palliative care

အကြိမ်ထောင်ချီ၊ မရေမနိုင်တဲ့ ဒီစကားဝိုင်းကို

to have that conversation thousands of times, countless times.

ကျွန်မရဲ့ သက်မွေးအလုပ်မှာ ဆက်လုပ်ခဲ့တယ်။

I always offer to stop.

ရပ်တန့်ဖို့ ကျွန်မ အမြဲကမ်းလှမ်းတယ်။

I've never been stopped.

ကျွန်မကို ဘယ်တော့မှ မရပ်တန့်စေခဲ့ဖူးဘူး။

But what happens at the end is that relaxation,

ဒါပေမဲ့ အဆုံးမှာ ဖြစ်သွားတာက စိတ်လက်အေးချမ်းတာပါ၊

that moment of, "Well, that isn't what I was expecting,"

"ကောင်းပြီ ၊ ဒါက ငါ မျှော်လင့်ထားတာ မဟုတ်ဘူး"ဆိုတဲ့ အခိုက်အတန့်ပါ၊

followed almost immediately by "Can you tell my family that?"

ချက်ချင်း နီးပါး လိုက်လာတာက "ဒါကို ငါ့မိသားစုကို မင်းပြောပေးလို့ရမလား၊

“Can you tell my wife, my husband, my kids, my parents?

ငါ့ဇနီး၊ ခင်ပွန်း၊ ကလေးတွေ၊ မိဘတွေကို မင်း ပြောပေးလို့ရလား။

That’s not what we’re expecting.”

ဒါက ငါတို့ မျှော်လင့်ထားတာ မဟုတ်ဘူး" ဆိုတာပါ။

And I think we can do that.

ကျွန်မတို့ ဒါကို လုပ်နိုင်တယ်လို့ ထင်တယ်။

So losing the wisdom really matters.

ဒါကြောင့် ဉာဏ်ပညာဆုံးၡုံးတာက တကယ့်ကို အရေးကြီးပါတယ်။

And we can't leave it to palliative care people,

ပျောက်ဆုံးသွားတဲ့ ဉာဏ်ပညာကို ပြန်ရယူဖို့ ရောဂါသက်သာရေး

or even medical people,

စောင့်ရှောက်သူတွေ၊ ဆေးဘက်ဆိုင်ရာ

to reclaim that lost wisdom, one family at a time.

လူတွေဆီမှာတောင် ဒါကို ချန်မထားနိုင်ဘူး၊ မိသားစုတစ်စု တစ်ကြိမ်ပေါ့။

This is a massive social public health issue.

ဒါက ဧရာမ အများပြည်သူ ကျန်းမာရေး ပြဿနာတစ်ခုပါ။

And I invite everybody who's listening to step up.

နားထောင်နေတဲ့ လူတိုင်းကို အရှိန်မြှင့်လုပ်ဆောင်ဖို့ ဖိတ်ခေါ်ပါတယ်။

The reason my grandmother understood about dying

သေတာနဲ့ပတ်သက်ပြီး ကျွန်မရဲ့ အဘွား နားလည်ခဲ့တဲ့ အကြောင်းရင်းက

was that she'd seen it alongside people who knew it,

သူက ဒါကို သိနေတဲ့ လူတွေအတူ၊ ဒီအကြောင်း

who described to her, as the process was happening,

သူ့ကို ပြောပြတဲ့ လူတွေအတူ၊ ဒါကို သူ မြင်ခဲ့တာပါ၊

what she was seeing,

သူ မြင်နေခဲ့တာက၊

so that she would understand and not be afraid.

ဖြစ်စဉ် ဖြစ်နေစဉ်မှာ၊ သူ ဒါကိုနားလည်ပြီး မကြောက်ရအောင်ပါ။

And it requires all of us who are mortals,

ပြီးတော့ ဒါက "ရပြီ၊ သေခြင်းတရားက

all of us who love other mortals,

ဆေးဘက်ဆိုင်ရာ ဖြစ်ရပ်တစ်ခုမဟုတ်ဘူး၊

to step up, to say, “Enough,

လူမှုရေး ဖြစ်ရပ်တစ်ခုပါ” လို့ ရှေ့ထွက်၊ ပြောဖို့

death is not a medical event, it's a social event.

ကျွန်မတို့ သေမျိုးအားလုံး၊ တခြားသေမျိုးတွေ ကို ချစ်တဲ့ ကျွန်မတို့အားလုံး လိုအပ်တယ်။

It's a deeply personal event.

ဒါဟာ လေးနက်တဲ့ ပုဂ္ဂိုလ်ရေး အဖြစ်အပျက်တစ်ခုပါ။

And we can understand it,

ကျွန်မတို့ ဒါကို နားလည်နိုင်တယ်၊

we can describe it,

ဒါကို ဖော်ပြနိုင်ပါတယ်၊

we can console each other.

အချင်းချင်း နှစ်သိမ့်နိုင်တယ်။

We can accompany each other.

အချင်းချင်း အဖော်ပြုနိုင်တယ်။

We can reclaim dying."

သေဆုံးတာကို ဆယ်တင်နိုင်ပါတယ်။"
